血清铁蛋白对军团菌肺炎的临床诊断价值

Study of serum ferreting for Legion Ella pneumonia diagnosis value

目的探讨血清铁蛋白诊断军团菌肺炎的临床价值。方法选取收治的疑似肺部感染的患者100例,经最后诊断分为军团菌肺炎组32例、典型细菌性病原体所致肺炎组52例、病毒性肺炎组6例及其他病原体肺炎组10例(包括衣原体、支原体、真菌等)。将各组患者的血红细胞沉降率、C反应蛋白、外周血血常规、血清铁蛋白等资料进行记录,对所有病例进行APACHEⅡ评分。使用化学发光法对血清铁蛋白进行检测。结果典型细菌性病原体所致肺炎组的细菌培养阳性率为85.7%;病毒性肺炎组细菌培养阳性率为0%;军团菌肺炎组细菌培养阳性率为26.1%;、其他病原体肺炎组细菌培养阳性率为20%。病毒性肺炎组和其他病原体肺炎组的铁蛋白中位数均比正常值低,典型细菌性病原体所致肺炎组的铁蛋白中位数比正常值略高,军团菌肺炎组患者的铁蛋白中位数较高于正常值。结论血清铁蛋白浓度升高可作为诊断军团菌感染的指标,可将其列为下呼吸道感染患者常规检测项目。

Objective To evaluate the serum ferreting diagnosis of Legion Ella pneumonia value. Methods 100 patients with suspected pulmonary infection patients were divided into the final diagnosis, Legion Ella pneumonia group, typical bacterial pathogens in pneumonia group, viral pneumonia group and other pathogens in pneumonia group （including Chlamydia, mycoplasma, fungi, etc. ）. The patients blood erythrocyte sedimentation rate, C-reactive protein, C of peripheral blood routine blood test, serum ferritin was recorded, APACHE II score in all cases. Detection of serum fer- reting was performed using a chemiluminescence method. Results The results of typical bacterial pathogens of pneumonia caused by groups of bacteria culture positive rate was 85.7%. The viral pneumonia group bacteria culture positive rate was 0%. The bacterial culture positive rate of Legion Ella pneumonia group was 26.1%. The positive rate of bacteria of the other pathogens in pneumonia group was 20%. The median ferreting typical bacterial pathogens in pneumonia group was slightly higher than the normal value, median ferreting Legion Ella pneumonia patients were higher than normal val ue. Conclusion The serum ferreting concentration can be used as a diagnosis of Legion Ella infection index, which can be classified as routine testing project with lower respiratory tract infection.
