检测Ⅱ型糖尿病糖调节正常一级亲属血清hs-CRP、TNF-α及IL-17的水平及意义 被引量:3

摘要 目的 探讨Ⅱ型糖尿病糖调节正常一级亲属血清中hs-CRP、TNF-α及IL-17的水平及影响胰岛素抵抗的相关危险因素.方法 采用前瞻性研究方法,收集2012年6月~2013年6月在体检中心确诊的Ⅱ型糖尿病患者的一级家属(子女、父母、兄弟姐妹)46例(一级亲属组)作为研究对象;选取同期健康体检的46例患者作为对照组,对两组患者的相关指标进行分析.结果 一级亲属组和对照组相关指标进行比较,其中BMI、HbA1c、TC、TG、HDL、hs-CRP、IL-17及TNF-α,差异有统计学意义(P<0.05);以胰岛素抵抗指标为应变量,将有显著统计学意义的变量BMI、HbAIc、TC、TG、HDL、hs CRP、I1-17、TNF-α等指标进行多因素非条件Logistic回归分析,结果 显示BM1、hs-CRP、IL-17、TNF-α是影响Ⅱ型糖尿病糖调节正常一级亲属胰岛素抵抗的主要危险因素.结论 hs-CRP、IL17、TNF-α在Ⅱ型糖尿病患者的一级家属呈高水平.BMI、hs-CRP、IL-17、TNF-α是影响Ⅱ型糖尿病糖调节正常一级亲属胰岛素抵抗的主要危险因素. Objective To explore the levels and significance and the related risk factors for insulin resistance of serum hs-CRP, IL-17 and TNF-α in first-degree relatives of type 2 diabetics. Methods By the prospective study method, 46 patients with type 2 diabetes were collected from June 2012 to June 2013 in hospital physical examination center. 46 patients with health check-up were selected as control group in the same period. The related indexes of two groups were analyzed. Results First-degree relatives group and the control group the related indexes were compared, including BMI, HbAlc, TC, TG, HDL, hs-CRP, IL-17 and TNF-α. The difference was statistically significant (P〈0.05). Many factors unconditional Logistic regression analysis revealed that BMI, HbAIc, TC, TG, HDL, hsCRP, IL-17 and TNF-α were main risk factors of insulin resistance. Conclusion The hs CRP, IL 17 and TNF alpha in first-degree relatives of type 2 diabetes have high level. BMI, HbAIc, TC,TG, HDL, hsCRP, IL-17 andTNF-α are main risk factors in first- degree relatives of type 2 diabetics.
