空气膨腹介质下腹腔镜胆囊切除术的临床应用价值

Clinical Application Value of Air Insufflation Medium under Laparoscopic Cholecystectomy

目的:探讨空气膨腹介质下腹腔镜胆囊切除术的疗效及安全性。方法:选取良性胆囊疾病患者146例,随机分为二氧化碳气腹组(对照组)和空气气腹组(观察组)。比较两组患者术前、术中及术后心率、血压、呼气末二氧化碳分压;比较两组患者术后排气时间、住院时间、治疗费用、术后疼痛程度及并发症情况。结果:两组患者术前、术中及术后心率、血压、呼气末二氧化碳分压比较差异均无统计学意义(P>0.05);观察组术后排气时间、疼痛程度、术后住院时间及住院费用等均优于对照组(P<0.05);两组术后主要并发症为术后肩痛、恶心呕吐及术后疼痛,观察组各并发症的发生率均低于对照组(P<0.05)。结论:空气气腹条件下腹腔镜手术的可行性较好,可减轻患者的疼痛程度,促进患者恢复,减少了治疗费用,值得临床深入研究。

Objective：To evaluate the efficacy and safety of laparoscopic cholecystectomy air insufflation medium.Method：146 patients with benign gallbladder were randomly divided into carbon dioxide pneumoperitoneum group（the control group） and air insufflation group（the observation group）.The heart rate,blood pressure,end tidal carbon dioxide partial pressure before,during and after the operation of two groups were compared.The exhaust time,hospitalization expenses,pain and complications after operation time of two gruops were compared.Result：There were no significant differences of the heart rate,blood pressure,end tidal carbon dioxide partial pressure before,during and after the operation between the two groups（P〉0.05）.The postoperative exhaust time,the degree of pain,postoperative hospitalization time and cost of hospitalization of the observation group were superior than those of the control group（P〈0.05）.The main postoperative complications were postoperative shoulder pain,nausea and vomiting,and postoperative pain.The incidence of complications of the observation group were lower than that of the control group（P〈0.05）.Conclusion：Air pneumoperitoneum laparoscopic operation can alleviate the pain of patients,promote the recovery of patients,reduce the cost of treatment,which is worth further clinical study.
