窄蒂脐旁穿支皮瓣与股前外侧穿支皮瓣修复手部小面积电烧伤的临床效果比较

Clinical analysis of the repairation of small area of electrical burns of hand by narrow by the umbilical perforators flap and femoral anterolateral perforators flap

目的比较窄蒂脐旁穿支皮瓣与股前外侧穿支皮瓣在修复手部小面积电烧伤时临床疗效。方法收集2011年3月-2013年12月手部小面积电烧伤患者204例,随机分为试验组和对照组各102例。接受常规电烧伤处理的基础上,试验组患者采用窄蒂脐旁穿支皮瓣修复,对照组患者采用股前外侧穿支皮瓣修复。术后随访6个月,比较2组患者皮瓣成活率、皮瓣色泽、外形、手部运动与感觉功能差异。结果术后随访6个月2组皮瓣移植成活率及创面Ⅰ期愈合的比例差异均无统计学意义(P>0.05);试验组皮瓣正常色泽、正常外形、正常感觉功能及正常运动功能的比例均高于对照组,差异均有统计学意义(P<0.05)。结论窄蒂脐旁穿支皮瓣和股前外侧穿支皮瓣修复存活率相近,但其远期移植效果更为显著。

Objective To discuss the curative effects of the repairation of small area of electrical burns of hand by narrow by the umbilical perforators flap and femoral anterolateral perforators flap. Methods 106 patients with small area of e-lectrical burns of hand,collected in the hospital during March 2011 to December 2013,randomly divided into experimental group and control group,each of 102 cases. On the basis of conventional electrical burn treatment,narrow by the umbilical per-forators flap was used to repair the experimental group,and femoral anterolateral perforators flap was used to repair the control group. Follow-up the patients 6 months after the surgery,compared the survival rate of skin flap,color,appearance of skin flap, the motor and sensory function of hand of two groups. Results Two groups of patients with skin flap after transplantation were followed up for 6 months,the statistical results showed that the survival rate of skin flap transplantation andⅠ wound healing effect of the two groups of patients has no statistical significance（P〉0. 05）,but the colour,appearance,motor and sensory function of skin flap of experimental group was more significant than the same indicators of control group（P〈0. 05）. Conclu-sion The survival rate of narrow by the umbilical perforators flap and femoral anterolateral perforators flap are similar,but the long-term transplant effect of narrow by the umbilical perforators flap is more pronounced.
