ICU老年患者中心静脉导管相关性血流感染的病原学分析

Analysis of Elderly Patients with ICU Central Venous Catheter Related Bloodstream Infection Pathogen

目的：分析本院老年危重患者中心静脉导管相关性血流感染的临床特点、菌群分布及药敏情况。方法回顾性分析2012年1月～2014年1月沈阳市第四人民医院ICU住院并留置深静脉导管的136例老年患者资料，计算导管相关性血流感染发生率，分析菌群分布及药敏情况。结果136例患者导管感染发生率为3.82‰；病原菌检出11株，分别为金黄色葡萄球菌3株、凝固酶阴性葡萄球菌1株、肺炎克雷伯杆菌2株、大肠埃希菌1株、铜绿假单胞菌1株、鲍曼不动杆菌1株、粘质沙雷菌1株、白色念珠菌1株。药敏结果提示6株革兰氏阴性菌耐药性较高，分离的革兰氏阳性菌为MRCN和MRSA，均对万古霉素敏感。结论在本次调查老年危重患者中心静脉导管相关性感染中，病原菌以G杆菌为主，药敏结果提示对常见抗菌药物的耐药性较高。

Objective Analysis of clinical characteristics of catheter-related bloodstream infection in critical y il elderly patients in our hospital, flora distribution and drug sensitivity. Methods A retrospective analysis of Fourth People＆#39;s Hospital of Shenyang city in 2012 January to 2014 January ICU hospitalization and deep vein indwel ing catheter in 136 cases of elderly patients with data, calculation of catheter-related bloodstream infection rate, analysis of flora distribution and drug sensitivity. Results The incidence rate was 3.82 per thousand in 136 cases of patients with catheter infection;pathogenic bacteria were detected in 11 strains were 3 strains of Staphylococcus aureus, 1 strains of coagulase negative staphylococcus, Klebsiel a pneumonia 2 strains, 1 strains of Escherichia coli, 1 strains of Pseudomonas aeruginosa, Acinetobacter, Bauman 1 strains of Serratia marcescens 1 strains, 1 strains of Candida albicans. The drug sensitivity results showed 6 strains of gram negative bacteriahad a high drug resistance, gram positive bacteria separation for MRCN and MRSA were sensitive to vancomycin. Conclusions In the center of infection in patients with the survey of critical elderlyvenous catheter-related, pathogenic bacteria with G-bacil i,Yao Minresults suggest for higher resistance to common antimicrobial agents.
