Experience of YU Ying'ao in the Treatment of Intractable Epilepsy

介绍余瀛鳌教授治疗难治性癫痫经验。认为本病病位主要在于脑、肝、脾,病机为脾虚酿痰,肝气郁结或肝阳上亢,挟痰上冲脑窍,脑络瘀阻,神机失用;病性实证多于虚证,热证多于寒证;病理要素以痰、瘀为主。临证主张辨病论治与辨证论治相结合,在通治法则指导下拟定潜镇止痫、化痰通络为主治法则,并自拟通治方"癫痫促效方"予以加减施治。

Experience of YU Ying＇ ao in the treatment of intractable epilepsy is introduced. The locations of epilepsy are brain,liver and spleen,and the pathogenesis is spleen,deficiency and accumulating phlegm,as well as liver qi stagnation,or hyperactivity of liver yang,so phlegm is brought up to the brain and blocking brain collaterals,hence occurs apraxia of the mind. As for the features,excess syndromes are more likely to be seen than deficiency syndromes and heat syndromes are more common than cold syndromes. Pathogenic factors are mainly phlegm and blood stasis. He proposed that treatments should be guided by syndrome and disease differentiation combined. Main treatment principle is suppressing exuberant yang to cease epilepsy,removing phlegm to unclog collaterals. Also Dianxian Cuxiao Decoction is used in practice.
