呼吸力学导向的机械通气策略 被引量:6

摘要 机械通气是各种原因所致呼吸衰竭非常重要的对症支持治疗手段,其极大程度地改善了患者预后.然而,不恰当的机械通气往往会引起呼吸机所致肺损伤(ventilator induced lung injury,VILI),因此恰当的制定通气策略是机械通气成功的关键因素.由于呼吸运动过程本身就是在中枢调控、神经反射等的调节下,呼吸肌肉收缩(或舒张)进而驱动气体吸入(或呼出)的力学过程.呼吸力学在机械通气中的意义越来越引起广大学者的关注.呼吸力学就是研究与呼吸运动有关的压力、容积和流量三要素及其相关的顺应性、阻力和呼吸做功等力学参数的一门科学.呼吸系统的各种疾病都会引起呼吸力学的改变,从而使呼吸运动发生异常.因此,呼吸力学是呼吸生理学的重要组成部分,也是机械通气的理论基础,是恰当的制定通气策略的重要依据.
