不同中医证型动脉粥样硬化与幽门螺旋杆菌感染的相关性研究

Correlation study between serum lipid,serum IL-6,TNF-α level and Helicobacter pylori infection of atherosclerotic patients with different TCM syndrome types

目的探讨幽门螺旋杆菌(Hp)感染对不同中医证型的动脉粥样硬化(AS)患者血脂和血清炎症因子的影响。方法通过临床流行病学的方法,在东直门医院收集120例AS患者及20例健康体检者,其中将AS患者分为肝肾阴虚、痰瘀互阻、气虚血瘀3组。空腹抽取静脉血,检测血脂、血清IL-6、TNF-α水平。结果血清Hp Ig G表达水平从高到低为肝肾阴虚组>气虚血瘀组>痰瘀互阻组>健康对照组(P<0.01)。AS组各中医证型患者血清Hp Ig G表达水平均显著高于对照组(P<0.01),肝肾阴虚组患者血清Hp Ig G表达水平与痰瘀互阻组组间差异具有显著统计学意义(P<0.01)。AS组患者胆固醇(TC)、IL-6、TNF-α水平均明显高于对照组,差异具有统计学意义(P<0.01)。AS患者血清Hp Ig G水平与TC、IL-6、TNF-α表达水平均成正相关关系(P<0.01)。结论 AS患者血清Hp Ig G水平明显高于对照组,AS患者血清Hp Ig G表达水平肝肾阴虚组高于痰瘀互阻组。Hp感染可能通过参与炎性因子的激活,干扰脂质代谢,从而促使AS的发生发展。

Objective To explore the effects of Helicobacter pylori （Hp） infection on blood lipid and serum inflammatory cytokines level in atherosclerotic （AS） patients with different TCM syndrome types.Methods Using clinical epidemiological method,120 AS patients and 20 cases of healthy check-up were recruited in Dongzhimen Hospital.AS patients were divided into the following groups：liver and kidney yin deficiency,phlegm and blood stasis trans-resistance,and qi deficiency and blood stasis groups.Their fasting venous blood was extracted.Their blood lipid,serum IL-6 and TNF α level were also measured.Results The expression level of serum Hp IgG in descending order was：liver and kidneyyin deficiency group ＞ qi deficiency and blood stasis group ＞ phlegm and blood stasis transresistance group ＞ control group （P ＜ 0.01）.Level of serum Hp IgG in all groups of AS patients was significantly higher than that of healthy controll group （P ＜0.01）.Moreover,level of serum Hp IgG in patients of liver and kidney yin deficiency group had a significant difference compared with Phlegm and blood stasis trans-resistance group （P ＜ 0.01）.Level of TC,IL-6 and TNF-α in AS patients was significantly higher than that of healthy controlled group （P ＜ 0.01）.The serum Hp IgG level of AS patients was positive correlation to the level of serum TC,IL-6 and TNF-α.Conclusion Level of serum Hp IgG in the AS patients was significantly higher than that of healthy control group.Level of serum Hp IgG in patients of liver and kidney yin deficiency group was higher than that in the patients of phlegm and blood stasis trans-resistance group.Helicobacter pylori infection might trigger AS by participating in activation of serum inflammatory cytokines and interfering with lipid metabolism.
