中青年肺部感染患者血清前白蛋白的变化

Changes of serum prealbumin in young and middle-aged patients with pulmonary infection

【目的】探讨中青年肺部感染患者血清前白蛋白水平变化,以期为该病的早期诊断和治疗提供依据。【方法】选取2012年2月-2013年2月我院住院的中青年肺部感染患者184例,同时随机抽取同期健康体检中青年患者50例为对照组,应用免疫透射比浊法和散射比浊法分别测定感染组和对照组血清前白蛋白和C反应蛋白水平。【结果】与对照组比较,肺部感染组患者血清前白蛋白水平明显降低[（0.177±0.073）g/L vs（0.264±0.063）g/L,P〈0.05];肺部感染组C反应蛋白与对照组比较其血清水平明显升高[（60.683±68.677）mg/L vs（3.522±1.847）mg/L,P〈0.05];根据年龄将肺部感染组患者分为〈40岁、40~50岁和51~59岁3个年龄组,不同年龄组患者血清前白蛋白水平差异有统计学意义[（161±0.067）g/L vs（0.173±0.075）mg/L vs（0.193±0.075）mg/L,P〈0.05]。Spearman相关分析显示,前白蛋白与C反应蛋白呈负相关（r=-0.487,P〈0.05）。【结论】中青年肺部感染患者血清前白蛋白水平明显降低,可能作为中青年肺部感染患者疾病诊断和病情监测的指标。

【Objective】To investigate the changes of serum prealbumin in young and middle-aged patients with pulmonary infection, so as to provide a basis for early diagnosis and treatment of pulmonary infection.【Methods】A total of 184 young and middle-aged patients with pulmonary infection hospitalized between January 2012 and January 2013 were entrolled as experimental group, at the same time 50 healthy cases randomly selected as control group. Serum prealbumin and c-reactive protein level were detected by project immune turbidimetry and scattering turbidimetry method.【Results】Compared with control group, serum prealbumin level of pulmonary infection group was significantly decresed [（0.177±0.073） g/L vs（0.264±0.063） g/L, P〈 0.05]. Compared with control group, serum CRP level of infection group was significantly incresed [（60.683±68.677） mg/L vs（3.522±1.847） mg/L, P〈0.05]. Dividing into groups according to age of patients（age 40,41~50, 51~59）, the serum prealbumin of three subgroups was significantly different [（0.161±0.067） vs（0.173±0.075） vs（0.193±0.075）, P 0.05]. According to correlation analysis, prealbumin was negatively correlated with creactive protein in infection patients（ r =-0.487, P〈0.05）.【Conclusion】The level of prealbumin in young and middle-aged patients with pulmonary infection is significantly decreased. Prealbumin may be used as an indicator for disease diagnosis and condition monitoring for young and middle-aged pulmonary infection patients.
