右美托咪定在鼻咽外科手术中的应用 被引量:1

摘要 目的:观察右美托咪定在鼻腔或咽部手术患者全身麻醉的应用效果及麻醉苏醒期的作用。方法:鼻、咽部行择期手术患者(ASAⅠ-Ⅱ)80例随机分为2组(n=40):对照组(C组)和右美托咪定组(D组),D组患者于插管前10 min予0.5μg/(kg·h)右美托咪定持续泵注至手术结束,C组给予等量同速的生理盐水。观察舒芬太尼注射后1 min内两组呛咳反应的程度与发生率,记录入室时(T0),插管前5 min(T1),插管即刻(T2)及插管后5 min(T3)心率(heart rate,HR)、收缩压(systolic blood pressure,SBP)、舒张压(diastolic blood pressure,DBP)及平均动脉压(meanarterial pressure,MAP)的变化情况及两组入麻醉恢复室后的拔管时间和拔管时MAP、HR、拔管后躁动评分等的比较。结果:两组拔管时间差异无统计学意义,与C组相比,D组呛咳反应的发生率降低(P〈0.05),严重心动过缓的发生率两组比较无统计学意义。插管时HR、SBP、DBP、MAP与均较插管前明显增加,D组变化程度显著低于C组(P〈0.01)。拔管时D组HR、MAP均低于C组(P〈0.05),拔管后躁动评分D组明显低于C组。结论:预注右美托咪定可有效减少舒芬太尼引起的呛咳反应的发生率,有效抑制气管插管所引起的心血管反应并可减少麻醉恢复期躁动的发生率,促进平稳恢复。
