肝胆胰恶性肿瘤患者游离DNA的含量及完整性的研究

摘要 目的探讨肝胆胰恶性肿瘤患者游离DNA的含量及完整性在临床诊断上的价值。方法收集肝胆胰恶性肿瘤患者血标本共43例(其中原发性肝细胞肝癌15例,胆道系统恶性肿瘤14例,胰腺癌14例),同期正常人血标本40例。检测短片段(ALU115)及长片段(ALU247)游离DNA的含量,并计算游离DNA的完整性(长片段含量/短片段含量)。结果恶性肿瘤患者游离DNA的ALU115、ALU247含量中位数分别为2.89 ng/μL及1.19 ng/μL,显著高于正常组0.85 ng/μL及0.28 ng/m L(P<0.001)。游离DNA的含量及完整性与肿瘤大小及分期、神经、脉管受侵无明显关联。ALU247 ROC曲线下面积为0.85,比短片段的0.80及完整性的0.56都高。结论游离DNA含量在肝胆胰恶性肿瘤患者与正常人有明显区别,且ALU247敏感性及特异性更高,可考虑作为肿瘤诊断的新指标。 Objective To investigate the clinical value of cell free DNA (ctDNA) content and DNA integrity. Methods Sera from 15 patients with HCC, 14 patients with biliary carcino- ma, 14 patients with pancreatic carcinomer and 40 healthy volunteers were analyzed by real time fluores-cence quantitative PCR(RTFQ-PCR) with 2 sets of primers (ALU 115 and ALU 247). DNA integrity was determined as a ratio of ALU247 to ALUll5. Results Median levels of ALU115 and ALU247 cfDNA in cancer patients were 2.89 ng/μL and 1.19 ng/μL, which were significantly higher than 0.85 ng/μL and 0.28 ng/μL in the control group. There was no signifi- cant difference in the integrity between two groups. Free DNA content and integrity were not correlated with tumor sizes, stage, nerve and vascular invasion. The area of ALU247 under ROC curve was 0.85. Conclusion The amount of cfDNA ALU247 is of higher sensitivity and specifici- ty to discriminate cancer patients from control, which may act as a new biomarker for tumor diag- nosis.
