弥漫性轴索损伤的磁共振成像新技术研究进展 被引量:1

摘要 弥漫性轴索损伤(diffuse axonal injury,DAI)是创伤性颅脑损伤(traumatic brain injury,TBI)中的一种特殊亚型,过去认为其是导致外伤患者严重致死致残或植物存活的主要原因[1]。但随着对其研究的深入,特别是超高场(3T)磁共振成像新技术包括结构成像技术[如磁敏感加权成像(susceptibilityweighted imaging,SWI)、磁共振弥散张量成像(diffusion tensor imaging,DTI)及纤维束示踪成像技术(fiber tractography,FT)]和功能成像技术[如磁共振波谱(magnetic resonance spectroscopy,MRS)、血氧水平依赖性blood.oxygen.1evel.dependent,BOLD)功能磁共振成像(functional magnetic resonance imaging,fMRI)及动脉自旋标记(arterial spin labeling,ASL)灌注成像]的进步和应用。提高了CT及常规MRI检查显示微小病灶和微结构损伤的检出率.可以发现传统成像技术难以显示的功能改变:不仅提高了对DAI疾病的诊断率(特别是增加轻中度DA!患者的检出),而且很大程度上改善了对DAI患者病理生理机制的认识。现笔者就较常见的此几种磁共振成像新技术在DAI的研究进展和应用进行综述。
