中期、晚期宫颈癌IGRT外放疗同步化疗效果分析

Effectiveness analysis of IGRT external radiotherapy With concurrent chemotherapy in 146 cases advanced cervical cancers

目的:探讨图像引导的放射治疗联合化疗在中、晚期宫颈癌患者治疗中的应用价值。方法:选取收治的中、晚期宫颈癌患者146例,均给予全程IGRT外放疗同步化疗,治疗完成后2个月评价其近期疗效,并随访观察毒副反应发生情况及不同临床分期患者3年、5年生存率。结果:146例患者中完全缓解(CR)138例,部分缓解(PR)6例,无效(SD)2例,没有出现进展(PD)病例。ⅡB期、Ⅲ期及Ⅳ期患者的3年存活率分别为100%(32/32),90.5%(19/21)及71.4%(10/14);5年生存率分别为81.8%(9/11),63.6%(7/11)及42.9%(3/7)。结论:IGRT可有效提高放射治疗的精确度,减少对正常组织的损伤,不良反应少且容易耐受,同步化疗可以最大程度杀灭肿瘤组织。

Objective To investigate the application of image - guided radiation therapy value in the treatment of patients with advanced cervical cancer. Method 146 patients with advanced cervicalcancer were prospectively enrolled in the study, and all were given IGRT external radiotherapy with concurrent chemotherapy and adjuvant chemotherapy two month later after treatment, follow - up the incidence of adverse reactions and 3 -year,5 -year survival rate of patients with different clinical stages. Results there was 138 cases of complete remission （CR） ,6 cases of partial remission （PR） and 2 cases of Invalid（SD） in the all 146 cases of patients. The 3 - year survival rate of patients were respectively 100% （32/32） ,90.5% （ 19/21 ） and 71.4% （10/14） in Ⅱ B, Ⅲ and Ⅳ Clinical stage, the 5 - year survival rate were respectively 81.8% （9/11 ） ,63.6% （7/11 ） and 42. 9% （3/7）. Conclusion IGRT can effectively improve the accuracy of radiation therapy, reduce the normal tissue damage ,fewer side effects and easily tolerated, and concurrent chemotherapy plus adjuvant chemotherapy can kill tumor tissue maximum extent.
