胃癌外科治疗的前提和基础是术前准确的分期,不同的分期其治疗理念不尽相同。内镜治疗逐渐成为早期胃癌的首选治疗方式,D2是进展期胃癌的标准手术。扩大手术范围并未使患者术后生存率有更多获益;多学科的共同参与,有效地将手术、化疗、放疗、分子靶向治疗等结合为一体是治疗进展期胃癌的最佳选择。

Accurate preoperative staging is imperative in surgical therapy of gastric cancer, disease in different stages mandate different management strategies. Endoscopic treatment has become the preferred treatment for patients with early gastric cancer. D2 radical gastrectomy is a standard surgical procedure for patients with advanced gastric cancer. Several clinical trials have revealed that more extended resection than D2 surgery has no impact on survival. Multimodality treatment in involving perioperative chemotherapy, radiotherapy or molecular targeting agents in addition to surgery have been thought to be promising treatment strategy for advanced gastric cancer.
