血清肌酶在假肥大型肌营养不良症的临床意义

摘要 目的:探讨血清肌酶异常升高对假肥大型肌营养不良症的早期诊断价值和基因携带者筛查的意义。方法:对3例假肥大肌营养不良症DuchenneMuscularDystrophy型(DMD)、1例BeckerMuscularDystrophy型(BMD)的临床资料及血清肌酶谱检验结果结合复习文献,进行分析。结果:4例患者,男性,临床体征Gowers征(+),3例DMD血清酶AST、ALT、LDH、HBDH平均分别为172U/L、315U/L、1103U/L、950U/L,约为正常人的5倍以上。CK、CKMB平均分别为11402U/L和433U/L,高达正常上限的60倍和30倍,CKMB/CK为3.8%,ALP、GGT、BUN、Gr、UA、血清离子及外周血象未见异常。1例BMD除CK为2207U/L, LDH、ALT、CKMB仅轻度增加。结论:假肥大肌营养不良症是X染色体连锁隐性遗传的儿童致死性神经肌肉系统疾病,血清生化最显著的改变是血中肌酶浓度异常升高,CK尤为明显。在可疑孕妇和幼儿中进行血清生化检验筛查,对假肥大肌营养不良症的早期诊断和基因携带者的检出有积极意义。
