硫酸庆大霉素注射疗法治疗肛瘘术后内口不愈大鼠模型的实验研究

Experimental study on gentamicin sulphate injection therapy for unhealed internal opening after anal fistula surgery in rat model

目的:观察注射疗法治疗肛瘘术后内口不愈大鼠模型的疗效。方法:选用SD大鼠80只,雌雄各半,进行肛瘘内口不愈模型制备后,随机分为4组,A组(硫酸庆大霉素组)、B组(空白组)、C组(生理盐水组)、D组(手术组),每组20只。A组每日在内口顶端及其潜在的瘘道侧壁上注射硫酸庆大霉素;B组不做任何处理;C组注射生理盐水;D组行传统的切开扩创引流术并每日生理盐水冲洗。分别于造模成功后第1天、治疗后第1、3、7、14天观察不愈内口创面分泌物、色泽、水肿情况,记录内口及创面完全愈合时间。结果:在不愈内口创面分泌物、色泽及水肿等评分方面,硫酸庆大霉素组均优于其他组,且内口完全愈合时间也均短于其他组。结论:硫酸庆大霉素注射疗法能改善伤口情况,从而促进肛瘘术后内口不愈大鼠模型肛瘘内口的愈合,缩短内口及创面完全愈合时间。

Objective ： To observe the therapeutic effect of injection therapy in the treatment of unhealed internal opening after anal fistula surgery. Methods ： Eighty Sprague - Dawley rats （ male - to - female ratio = 1 ： 1 ） were estab- lished as an unhealed internal opening model and were equally divided into group A（ gentamicin sulphate group） , group B （blank group）, group C （normal saline group）, and group D （surgery group）. Group A was given gentamicin sulphate injection at the top of the internal opening as well as the potential side wall of fistula ; group B was given no treatment;group C was injected with normal saline;group D was given traditional incision,debridement,and drain- age, as well as daily irrigation with normal saline. The secretions, color, and edema of the unhealed internal opening were observed one day after model establishment and 1,3,7 and 14 days after treatment. The healing time of inter- nal opening and wound surface in each group was recorded. Resuhs：Compared with the other groups,the gentamicin sulphate group bad significantly higher scores in secretions, color, and edema of the unhealed internal opening, and had a significantly shorter healing time of the internal opening as well. Conclusion： Gentamicin sulphate injection therapy can improve wound healing, and thus promote the healing of the internal opening after anal fistula surgery in rat model and shorten the healing time of the internal opening and wound surface.
