高频彩色超声诊断甲状腺微小乳头状癌 被引量:5

摘要 目的探讨甲状腺微小乳头状癌(papillary thyroid microcarcinoma,PTMC)的超声声像图特征,以提高术前超声诊断的准确率。方法回顾性分析69例患者(91个最大直径≤10mm甲状腺结节)的二维灰阶超声和彩色能量多普勒声像图特征,并与术后病理相对照。术前均测量结节的最大横径值和最大纵径值,并计算横纵径比值(transverse diameter-to-longitudinal diameter ratio of thyroid nodules,T/L);观察结节内部回声、形态、边界、有无钙化等;然后应用彩色能量多普勒观察结节内有无彩色血流及分布。结果 PTMC典型声像图特征为:以实质性低回声为主,形态多不规则,边界多不清晰,T/L≥1,多数结节内有微钙化,内部可见到较丰富的血流信号(P<0.01)。高频彩色超声诊断PTMC的敏感度、特异度和准确度分别为86.67%、89.13%和87.91%。结论高频彩色超声对PTMC的早期诊断具有重要价值。
