摘要 目的探讨改变ICU病房结构对鲍曼不动杆菌院内感染的影响效果。方法查阅本院2012年6月-2014年6月ICU病房收治的1241例病例资料,按照入院时间不同进行分组,其中2013年6月-2014年6月收治的652例患者作为研究组,2012年6月-2013年6月收治的589例患者作为对照组,对照组仅根据以往工作经验对ICU病房结构进行布局,研究组对ICU病房结构进行适当调整,目的在于有效降低ICU病房患者发生鲍曼不动杆菌院内感染率。记录两组ICU病房患者发生鲍曼不动杆菌院内感染率、住院时间、治疗费用、因鲍曼不动杆菌院内感染所致死亡率、患者或家属满意度等,给予统计学分析并得出结论。结果研究组鲍曼不动杆菌院内感染率仅为1.69%、因鲍曼不动杆菌院内感染致死率仅为0.15%,显著低于对照组的14.09%、2.89%(P〈0.05);研究组的住院时间短于对照组,治疗费用低于对照组,满意度评分高于对照组(P〈0.05)。结论合理改变ICU病房结构可显著降低鲍曼不动杆菌院内感染发生率,有利于患者尽快恢复健康,保障其生活质量及生命安全,利于维持良好的医患关系。 Objective To probe into the impact effect of ward structure changing in intensive care unit(ICU)on nosocomial infection due to Baumanii. Methods 1241 datum of medical cases admitted into ICU from June 2012 to June 2014 were looked up in our hospital.According to different admission time,they were divided.652 patients from June 2013 to June 2014 admitted into our hospital were selected as research group and the rest 589 patients treated in ICU from June 2012 to June 2013 were categorized as control group.In the control group,ICU ward structure was arranged only based on working experience in the past,while in the research group,ICU ward structure was adjusted in an appropriate way aiming at effectively decreasing the rate of nosocomial infection due to Baumanii.The Baumanii-induced hospital infection rate,hospital stay,cost,mortality rate due to Baumanii-induced hospital infection,satisfaction by patients or their relatives were recorded and statistically analyzed. Results In the research group,the rate of Baumanii-induced hospital infection was only 1.69%,and the mortality rate due to Baumanii-induced hospital infection was only 0.15%,much lower than that in the control group accounting for 14.09% and 2.89% respectively(P〈0.05).The hospital stay in the research group was shorter than that in the control group,cost in the research group was less than that in the control group,and satisfaction score in the research group was higher than that in the control group(P〈0.05). Conclusion Appropriate change of ICU ward structure can remarkably decrease the occurrence of Baumanii-induced hospital infection,and make for patient′s quick recovery,guarantee the quality of life and life safety,which is beneficial to hold a good doctor-patient relationship.
