绝经前后女性2型糖尿病患者氧化低密度脂蛋白的变化及相关影响因素的分析 被引量:4

Analysis on the changes of oxidized low- density lipoprotein level and the influencing factors in premenopausal and postmenopausal women with type 2 diabetes mellitus

摘要 目的:探讨绝经前后女性2型糖尿病(T2DM)患者氧化低密度脂蛋白(ox-LDL)水平的变化及相关影响因素的分析。方法:选取2013年9月-2014年3月在该院住院的女性T2DM患者117例为T2DM组,另选取体检科糖耐量正常的女性83例为对照组。测定两组ox-LDL、血脂、血压、空腹血糖(FBG)、体质指数(BMI)和肾功能等指标,分析ox-LDL与其他指标的相关性。结果:对照组绝经后女性年龄及甘油三酯(TG)、总胆固醇(TCH)、收缩压(SBP)、舒张压(DBP)、BMI、r-谷氨酰转肽酶(r-GT)、尿素氮(BUN)、血尿酸(UA)水平均高于绝经前,差异有统计学意义(P〈0.05)。T2DM组绝经后年龄及女性低密度脂蛋白胆固醇(LDL-C)、SBP、BMI、BUN、UA、ox-LDL、FBG水平均高于绝经前,差异有统计学意义(P〈0.05)。T2DM组绝经前TG、LDL-C、SBP、DBP、BMI、FBG、谷丙转氨酶(ALT)、r-GT及ox-LDL水平高于对照组,差异有统计学意义(P〈0.05);高密度脂蛋白胆固醇(HDL-C)低于对照组,差异有统计学意义(P〈0.05)。T2DM组绝经后年龄及TG、LDL-C、SBP、FBG、谷草转氨酶(AST)、ALT、r-GT、ox-LDL水平高于对照组,差异有统计学意义(P〈0.05);HDL-C低于对照组,差异有统计学意义(P〈0.05)。通过多元线性回归分析得出,HDL-C、UA、肌酐(SCr)是ox-LDL的独立影响因素。结论:绝经前后的T2DM女性ox-LDL水平升高,绝经后较绝经前升高更明显。ox-LDL水平与UA、SCr和HDL-C水平密切相关。 Objective: To explore the changes of oxidized low- density lipoprotein( ox- LDL) level and the influencing factors in premenopausal and postmenopausal women with type 2 diabetes mellitus( T2DM). Methods: A total of 117 T2 DM women were selected from this hospital from September 2013 to March 2014 as T2 DM group,83 women with normal glucose tolerance after physical examination were selected as control group. The detection indexes included ox- LDL,blood lipids,blood pressure,fasting blood glucose( FBG),body mass index( BMI) and renal function; the correlation between ox- LDL and the other indexes was analyzed. Results: In control group,age,triglyceride( TG),total cholesterol( TC),systolic blood pressure( SBP),diastolic blood pressure( DBP),BMI,γ- glutamyltranspeptidase( r- GT),blood urea nitrogen( BUN) and uric acid( UA) levels in postmenopausal women were statistically significantly higher than those in premenopausal women( P〈0. 05). In T2 DM group,age,low density lipoprotein cholesterin( LDL- C),SBP,BMI,BUN,UA,ox- LDL and FBG levels in postmenopausal women were statistically significantly higher than those in premenopausal women( P〈0. 05). Among premenopausal women,TG,LDL- C,SBP,DBP,BMI,FBG,alanine aminotransferase( ALT),r- GT and ox- LDL levels in T2 DM group were higher than those in control group,while high density lipoprotein cholesterin( HDL- C) level in T2 DM group was lower than that in control group,there were statistically significant differences( P〈0. 05). Among postmenopausal women,age,TG,LDL- C,SBP,FBG,aspartate aminotransferase( AST),ALT,r- GT and ox- LDL levels in T2 DM group were higher than those in control group,while HDL- C level in T2 DM group was lower than that in control group,there were statistically significant differences( P〈0. 05).Multivariate linear regression analysis showed that HDL- C,UA and creatinine were independent influencing factors of ox- LDL. Conclusion: ox- LDL levels increase in premenopausal and postmenopausal women with T2 DM,especially in postmenopausal women. ox- LDL level is closely correlated with UA,SCr and HDL- C levels.
