脂肪干细胞在创面愈合中的研究进展及应用前景 被引量:9

摘要 创伤面积大、部位深、炎性反应剧烈等因素导致创面无法尽早愈合,是目前临床上所面临的严峻挑战.再生医学利用人体干细胞以及细胞因子发挥治疗效应,已经成为一种治疗创伤的选择性手段,然而人体干细胞来源缺乏直接限制了再生医学的进一步发展.脂肪干细胞(ASCs)的出现,很大程度解决了这个难题,甚至有望在组织再生领域发挥重大作用.ASCs是在脂肪组织的血管基质片段(SVF)中发现的一类间充质干细胞,源于中胚层,其来源丰富且易于分离,具有多向分化(成脂、成骨、成软骨等)能力以及旁分泌、免疫调节等功能,适用于多种治疗用途.早期研究认为ASCs的治疗作用是分化为组织相关细胞,通过直接替代作用参与组织修复.但近年来越来越多的研究更倾向于认为ASCs的治疗作用主要是通过发挥旁分泌和免疫调节功能,调控组织周围相关细胞以及改善微环境等作用参与修复和重建,而不是直接替代作用[1-2].因此,现从ASCs的旁分泌以及免疫调节两方面说明ASCs在创面愈合的作用,并进一步阐述其应用前景.
