溶栓后血压管理在急性脑梗死治疗中的作用

Effect of blood pressure management on acute cerebral infarction treatment after thrombolysis

目的探讨溶栓后血压管理在急性脑梗死治疗中的作用情况。方法收集抚顺市中心医院2011年2月~2014年6月收治的120例急性脑梗死患者临床资料,依据是否进行溶栓后的血压管理进行分组,血压管理组（60例）和对照组（60例）。观察并比较两组急性脑梗死患者神经功能缺损评分和临床治疗效果情况。结果对照组急性脑梗死患者治疗前神经功能缺损评分[（19.7±4.3）分]和血压管理组[（19.2±4.8）分]比较,差异无统计学意义（t=0.60,P〉0.05）;治疗后血压管理组患者神经功能缺损评分[（8.6±1.3）分]优于对照组[（10.7±1.9）分],差异有统计学意义（t=7.07,P〈0.05）。血压管理组急性脑梗死患者临床治疗总有效率（90.0%）优于对照组（70.0%）,差异有统计学意义（χ^2=13.86,P〈0.05）。急性脑梗死患者溶栓后血压管理组溶栓后梗死灶继发出血率（0.0%,0/60）明显低于对照组（8.3%,5/60）,差异有统计学意义（χ^2=7.35,P〈0.05）。结论急性脑梗死溶栓后实施血压管理可以有效促进神经功能恢复,提高治疗效果,值得临床推广应用。

Objective To discuss blood pressure management on acute cerebral infarction treatment after thrombolysis. Methods From February 2011 to June 2014, the clinical data of 120 patients with acute cerebral infarction in the Central Hospital of Fushun City were selected and divided into two groups, control group （60 cases） and blood pressure management group （60 cases）, according to whether Blood pressure management or not after thrombolysis. The NIHSS and clinical curative effect score of two groups were observed and compared. Results Before the treatment, the NIHSS score of control group [（19.7±4.3） scores] and blood pressure management group [（19.2±4.8） scores] were compared, the difference was not statistically significant （t = 0.60, P〉 0.05）; after the treatment, the NIHSS score of blood pressure management group [（8.6±1.3） scores] was better than that of control group [（10.7±1.9） scores], the difference was statis-tically significant （t=7.07, P〈0.05）. Total clinical curative effective rate of blood pressure management group （90.0%） was better than that of control group （70.0%）, the difference was statistically significant （χ^2=13.86, P〈0.05）. Infarcts secondary bleeding after thrombolysis of blood pressure management group （0.0%, 0/60） was lower than that of control group （8.3%, 5/60）, the difference was statistically significant （χ^2=7.35, P 〈 0.05）. Conclusion The blood pressure management on acute cerebral infarction treatment after thrombolysis can promote neurological function recovery, im-prove treatment effect, and it is worth to be used.
