摘要
目的检测Ki67抗原和基质金属蛋白酶(MMP-2)在胆囊癌及慢性胆囊炎组织中的表达,探讨两者与胆囊癌的增殖、侵润及转移的关系。方法应用免疫组化SP法检测Ki67和MMP-2在52例胆囊癌患者的癌组织及21例慢性胆囊炎患者的胆囊组织中的表达情况。结果 Ki67和MMP-2在胆囊癌组织中阳性表达率分别为84.6%和78.8%;在慢性胆囊炎胆囊组织中的阳性表达率分别为42.9%和38.1%,差异均有统计学意义(P<0.05)。Ki67的阳性表达与胆囊癌的分化程度、Nevin分期有关(P<0.05),MMP-2的阳性表达与胆囊癌的分化程度、Nevin分期及淋巴结转移有关(P<0.05)。两者均与患者的年龄、性别及肿瘤的大小无相关性(P>0.05)。Ki67与MMP-2在胆囊癌组织中的阳性表达呈正相关(r=0.427,P=0.002)。Ki67和MMP-2表达阴性的患者术后生存期高于表达阳性的患者。结论 Ki67和MMP-2的阳性表达与胆囊癌的增殖和浸润有关,联合检测Ki67和MMP-2,在一定程度上有助于判断胆囊癌进展及预后。
Objective To investigate the expressions of Ki67 antigen and matrix metallo protinase-2 (MMP-2) in the gallbladder carcinoma and chronic cholecystitis tissue,and to analyze their correlation with proliferation,invasion,metastasis of gallbladder carcinoma.Methods SP immunohistochemistry was used to determine the expression of Ki67 and MMP-2 in 52 gallbladder carcinomas,21 chronic cholecystitistissue.Results The positive rate of Ki67 and MMP-2 in gallbladder carcinomas are respectively 84.6%,78.8%.significandy higher than chronic cholecystitis (Ki67:42.9%,MMP-2:38.1%,P < 0.05).The expressions of Ki67 were correlated with the tumor differentiation,Nevin stages (P < 0.05),and the expressions of MMP-2 were correlated with the tumor differentiation,Nevin stages and Lymph node Metastasis (P < 0.05),both of them are not with the patient,s age,gender and tumor size (P > 0.05).Ki67 antigen expression was positively correlated with matrix metallo protinase-2 (MMP-2) expression in gallbladder carcinoma(r =0.427,P =0.002).Patients with negative expression of Ki67 and MMP-2 had longer survival time than those with positive expression of Ki67 and MMP-2.Conclusion The expression of Ki67 and MMP-2 was associated with proliferation,invasion,metastasis of gallbladder carcinoma,combined detection of Ki67 and MMP-2 is conducive to judging the progress and prognosis of the gallbladder carcinoma.
