复合式小梁切除术治疗原发性闭角型青光眼的可行性研究

Feasibility study of composite trabeculectomy in the treatment of primary angle-closure glaucoma

目的探讨复合式小梁切除术治疗原发性闭角型青光眼的临床效果。方法选取2009年6月-2013年6月本院收治的24例原发性闭角型青光眼患者作为研究对象,随机分为观察组和对照组,各12例。观察组采用复合式小梁切除术治疗,对照组采用传统小梁切除术治疗。比较两组不同时间的眼压、功能性滤泡形成率及并发症发生率。结果两组治疗后3 d、1周、1个月的眼压比较,差异无统计学意义（P〉0.05）;观察组治疗后3个月、6个月及1年的眼压显著低于对照组,差异有统计学意义（P〈0.05）。观察组的功能性滤泡形成率为83.33%,显著高于对照组的58.33%,差异有统计学意义（P〈0.05）。两组的并发症发生率比较,差异有统计学意义（P〈0.05）。结论复合式小梁切除术治疗原发性闭角型青光眼的效果显著,值得临床推广应用。

Objective To explore clinical effect of the composite trabeculectomy in the treatment of primary angle-closure glaucoma. Methods 24 cases with primary angle-closure glaucoma from June 2009 to June 2013 in our hospital were selected and randomly divided into the observation group and the control group,12 cases in each group.The observation group was treated with composite trabeculectomy,the control group was treated with traditional trabeculectomy.Intraocular pressure in different time,the formation rate of functional follicular membrane and the incidence of complications was compared respectively. Results Intraocular pressure at 3 d,1 week after treatment between two groups,there was no significant difference（P〉0.05）.Intraocular pressure at 3 months,6 months and 1 year after treatment in the observation group was lower than that in the control group,with significant difference（P〈0.05）.The formation rate of functional follicular membrane in the observation group was 83.33%,which was higher than 58.33% in the control group,with significant difference（P〈0.05）.The incidence of complications between two groups,there was significant difference（P〈0.05）.Conclusion The clinical effect of the composite trabeculectomy in the treatment of primary angle-closure glaucoma is obvious,it is worthy of clinical promotion and application.
