Seldinger置管法腹腔引流术治疗重症急性胰腺炎的临床效果观察

Clinical efficacy observation of Seldinger catheter method peritoneal drainage in treatment of severe acute pancreatitis

目的探讨Seldinger置管法腹腔引流术治疗重症急性胰腺炎的临床效果。方法根据治疗方法的不同,将本院2012年4月-2014年4月收治的60例重症急性胰腺炎患者分为研究组和对照组各30例,对照组实施常规治疗,研究组在对照组基础上行Seldinger置管法腹腔引流术。观察、记录研究组患者的术中出血量、手术时间、腹内压下降、排气时间等,比较两组的腹痛缓解时间、住院时间以及并发症发生率。结果研究组患者的腹痛缓解时间、住院时间均明显短于对照组,差异有统计学意义（P〈0.05）;研究组的腹腔感染、器官衰竭、胰腺脓肿、假性囊肿、急性液体积聚的发生率及中转手术率均显著低于对照组,差异有统计学意义（P〈0.05）。结论 Seldinger置管法腹腔引流术治疗重症急性胰腺炎具有方法简便、创伤小、引流效果好、并发症少等优势,是非手术治疗重症急性胰腺炎的理想方法 。

Objective To investigate the clinical efficacy of Seldinger catheter method peritoneal drainage in the treatment of severe acute pancreatitis. Methods According to different treatments,60 cases of patients with severe acute pancreatitis treated in our hospital from April 2012 to April 2014 were divided into research group（n=30） and control group（n=30）,the conventional treatment was taken to the control group,Seldinger catheter method peritoneal drainage was added to the research group on the basis of control group.The blood loss,operative time,intra-abdominal pressure drop,exhaust time of research group was recorded,the time of abdominal pain,hospital stay and the incidence of complication in the two groups was compared. Results The time of abdominal pain,hospital stay of research group was shorter than that of control group,the difference was significant（P〈0.05）;The incidence of abdominal infections,organ failure,pancreatic abscess,pseudocyst,acute fluid accumulation,and the rate of surgical intervention in the research group was lower than that of the control group,the difference was significant（P〈0.05）. Conclusion Seldinger catheter method peritoneal drainage in the treatment of severe acute pancreatitis has the advantages of simple,less invasive,good drainage effect,fewer complications,is the ideal way to treat severe acute pancreatitis by non-surgical treatment.
