基于契约理论的家庭医生式服务支付机制设计

Design of A Contract Theory-based Family Doctor-service Payment Mechanism

近年来家庭医生式服务在我国许多地区陆续开展,但由于家庭医生服务能力有限、缺乏专项补助、相关配套政策不完善等问题,家庭医生式服务的进一步推广和普及受到了阻碍。文章从契约理论的角度出发分析家庭医生式服务支付过程中参与各方的关系,设计一种具有激励和约束各方行为作用的支付方式,从而达到提高医保基金的保障绩效、激励家庭医生提高服务效率的目的。

Family doctor-contract-service has been consecutively implemented in many areas of China recently. While the distribution and popularization of this service was restricted by some reasons including the limited service ability of family doctors, the lack of specific grants and the short of related supporting policies, etc. The aims of this study were to analyze the interactions among participants of family doctor-service payment,to devise a payment method that could motivate and restrict the behaviors of partici- pants and finally to improve guarantee performance of medical insurance fund and service efficiency of family doctor.
