无锡市社区高血压、糖尿病患者疾病经济负担调查研究

Economic Burden of Patients with Hypertension or Type 2 Diabetes Mellitus in Wuxi' Communities

目的了解无锡市高血压病和糖尿病患者的疾病经济负担,为有效控制慢性病患者医疗费用、制定社区慢性病管理策略提供科学依据。方法对随机抽取的社区高血压病和糖尿病患者开展关于卫生服务利用情况和疾病医疗费用支出的问卷调查。结果完成调查的593名无锡市社区高血压、糖尿病患者,药物治疗依从性较好(92.24%),社区卫生服务中心(站)是社区高血压、糖尿病患者最主要的药物来源(53.4%),也是利用门诊服务的最主要医疗卫生机构(就诊率54.3%),调查结果显示无锡市社区高血压、糖尿病患者的门诊、住院服务需求基本得到了满足。无锡市高血压、糖尿病患者年直接经济负担约79.0亿元,占无锡市2012年GDP(7568.15亿元)的1.04%,而疾病间接经济负担较轻。结论无锡市高血压、糖尿病疾病经济负担沉重,应当加强社区慢性病管理,以有效控制病程、降低疾病经济负担。

Objective To investigate the economic burden of community patients with hypertension or diabetes, and to provide the references for reducing the medical expenses and establishing the Wnxi model of chronic diseases management. Methods Patients with hypertension or diabetes in Wuxi＇s communities which were randomly extracted were investigated on the medical expenses and health service utilization. Results Totally 593 patients who completed the questionnaire, had high medication adherence （ 92.24% ）. The proportion of the patients who got medicine from community health service center was 53.4%, meanwhile the proportion of outpatients who got health service from community health service center was 54.3%. The results showed that the requirement for health service of patients with hypertension or diabetes in Wuxi＇s communities was mainly met. Every year, hypertension and diabetes cost 7.90 billion RMB for direct economic burden,about 1.04% of Gross Domestic Product at the same period,however, the indirect economic burden were not heavy. Conclusions Economic burden caused by hypertension and diabetes in Wuxi was heavy. Community chronic diseases management model should be effectively contributed for reducing the economic burden.
