老年慢性肾衰竭患者病情恶化的危险因素分析及干预对策

Analysis on the Risk Factors of Disease Progression in Elderly Patients with Chronic Renal Failure and Intervention Strategies

目的探讨老年慢性肾衰竭患者病情恶化的危险因素,并采取相对应的处理措施,以期延缓病情进展。方法回顾性分析2010年1月至2013年1月武汉市中心医院收治的70例老年慢性肾衰竭患者病例资料,入院前排除病情恶化者,入院后对病情恶化者予以干预,对其中病情恶化患者进行危险因素分析。结果合并3种以上基础疾病、严重感染、血压控制不良、水电解质紊乱、原发病加重、心力衰竭是老年慢性肾衰竭患者病情恶化高危险因素(均P<0.05),而与患者的年龄、性别无关(P>0.05)。干预后有14例患者病情明显好转,总有效率为73.68%。结论老年慢性肾衰竭患者病情恶化的危险因素多样,及时控制感染,加强原发疾病的整体治疗,对于控制病情的进展有重要作用。

Objective To investigate the risk factors of disease progression in elderly patients with chronic renal failure,and corresponding measures to delay disease progression.Methods Clinical data of 70 elderly patients with chronic renal failure admitted in Wuhan Central Hospital from Jan.2010 to Jan.2013 were retrospectively analyzed,the patients had been excluded deteriorated ones before admission,and after admission deteriorated patients received intervention,risk factors of the disease were analyzed basing on such patients.Results The high-risk factors of the deterioration of chronic renal failure in elderly patients included three or more underlying diseases combined,severe infections,poorly controlled blood pressure,fluid and electrolyte disorders,primary disease exacerbation,heart failure （P 〈 0.05）,and it had nothing to do with the patient＇s ageand gender（P 〉 0.05）.Mter the intervention,condition of 14 cases wasimproved markedly,the total effective rate was 73.68%.Conclusion Risk factors of the deterioration of chronic renal disease in elderly patients are various,timely controlling the infection and enhancing the overall treatment for the primary disease have great importance in controlling the progression of the disease.
