摘要
目的:探讨螺旋CT三维成像检查在踝关节创伤性骨折中的应用。方法:收集2012年10月-2013年4月期间于我院就诊的踝关节创伤性骨折患者110例相关资料,分别进行X线、CT扫查及三维成像分析,对比三种方式对踝关节创伤性骨折的诊断情况。结果:X线诊断踝关节骨折86例,符合率78.2%;CT轴位诊断踝关节骨折100例,符合率90.9%;CT轴位+三维重建诊断踝关节骨折110例,诊断符合率100.0%。CT三维重建的诊断符合率明显高于CT轴位和X线,其中与X线检查结果具有显著性差异(P<0.05)。X线检查结果显示无踝关节软骨骨折;CT轴位显示软骨骨折48例,诊断率81.3%;三维重建显示踝关节软骨骨折59例,诊断率100.0%。三维重建对软骨骨折的诊断率显著高于CT轴位和X线,差异具有显著性(P<0.05,P<0.01)。结论:螺旋CT三维重建能够克服X线、CT轴位图像的缺点,更准确地显示骨折部位、数目等,对于合并的软骨骨折也有更高的诊断率,可将其作为临床诊疗的重要依据。
Objective: To investigate the application of spiral CT imaging in traumatic ankle fracture. Methods:October 2012-anlde fracture treatment of patients with traumatic April 2013 in our hospital 110 cases during the relevant information, namely X-ray, CT scanning and three-dimensional imaging analysis, comparison of three methods for traumatic ankle diagnosis of the situation fracture. Results:Diagnosis of ankle fracture 86 cases by X-ray, in line with the rate of 78.2% ;CT axial diagnosis of ankle fractures 100 cases, in line with the rate of 90.9% ;CT reconstruction axial+110 cases of ankle fracture diagnosis,diagnosis rate of 100.0%. Diagnostic CT reconstruction compliance rate was significantly higher than axial CT and X-ray, X-ray examination and the result of which was a significant difference (P〈0.05). X-rays showed no fracture of the ankle cartilage; CT axial Show cartilage fracture in 48 cases,the diagnosis rate of 81.3%; displays three-dimensional reconstruction of fracture of ankle cartilage in 59 cases,the diagnosis rate of 100.0%. Three-dimensional reconstruction of cartilage fracture diagnosis was significantly higher axial CT and X-ray,and the difference was significant (P〈0.05,P〈0.01). Conclusion:Spiral CT X-ray can overcome the disadvantage axial CT imagos,a more accurate display of the fracture site,number, etc., for the consolidation of cartilage fracture have a higher diagnostic yield,which can be used as an important basis for clinical diagnosis and treatment.
