血流向量成像技术在梗阻性肥厚型心肌病左心室收缩功能评价的应用研究 被引量:3

摘要 目的:观察血流向量成像(VFM)技术在梗阻性肥厚型心肌病左心室收缩功能评价的应用效果。方法:将31例确诊为左室流出道梗阻的肥厚型心肌病(HCM)患者作为观察组,选择40名健康体检者为对照组,分别记录两组心尖五腔心切面基底水平、乳头肌水平和心尖水平取样线上收缩期峰值速度(Vs)、峰值流量(Fs)、收缩期负向总流量(SQ)以及左心室收缩期3个时相等容收缩期、快速射血期及减慢射血期涡流的横径与纵径,各项数据采用SPSS17.0进行统计分析处理。结果:两组常规超声心动图在LAD、LVDD及LVEF等比较均具有统计学意义(t=5.133,t=4.732,t=8.622;P<0.05)。在基底水平、乳头肌水平和心尖水平3个切面上测定Vs、Fs、SQ,两组比较均具有显著差异。左心室收缩期3个时相等容收缩期、快速射血期及减慢射血期涡流的横径与纵径比较均具有统计学意义。结论:VFM技术能够有效鉴别梗阻与非梗阻性肥厚型心肌病,能够有效评估患者的左室收缩期功能,为临床诊断提供量化的参考依据。 Objective: To observe the vector flow mapping in hypertrophic obstructive cardiomyopathy evaluation of left ventricular systolic function. Methods:Selected 31 patients as the observation group who was diagnosed as left ventricular outflow tract obstruction in HCM with hypertrophic cardiomyopathy in Meihekoushi Central Hospital, between January 2012 to February 2014,and selected the concomitant healthy 40 patients served as controls group, two groups of patients were recorded apical five-chamber view of the base level of the papillary muscle level and apical horizontal sampling line peak systolic velocity, peak flow, systolic negative total flow, and left ventricular systolic equal to three time systolic volume, rapid ejection period, slowing ejection of the vortex diameter and longitudinal diameter, the results were statistically analyzed using SPSS17.0 treatment. Results: Two groups of patients compared conventional echocardiography in LAD, LVDD, LVEF, etc., were statistically significant (t=5.133, t=4.732, t=8.622; P〈0.05). Determination of Vs, Fs, SQ on the base level, the level of the papillary muscle level and apical three section, the two groups were statistically significant. Systolic left ventricular systolic volume 3 are equal, rapid ejection period, slowing ejection of the vortex diameter and longitudinal diameter comparison was statistically significant. Conclusion: VFM can effectively identify obstructive and non-obstructive hypertrophic cardiomyopathy, left ventricular systolic function effectively assess patients, can provide a quantitative reference for clinical diagnosis.
