摘要
目的:探讨不同年龄组患者烤瓷冠修复后龈沟液成分的变化情况。方法:选择2012年4月至2014年1月在解放军总医院口腔科就诊,需进行烤瓷单冠修复的患者100名作为受试对象,其中男、女性各50名,年龄20-59岁,按年龄段分成20-29岁组,30-39岁组,40-49岁组,50-59岁组,每组25名。采用钴铬合金烤瓷单冠进行修复,分别在修复前及修复后1个月检测基牙龈沟液的分泌量以及白细胞介素-1β(interleukin-1β,IL-1β),白细胞介素-6(inter leuk in-6,IL-6)和肿瘤坏死因子-α水平(tumor necr osis factor-α,TNF-α)。结果:修复后1个月基牙龈沟液分泌量以及白细胞介素1,白细胞介素6和肿瘤坏死因子α水平高于修复前。但随着年龄增加,龈沟液分泌量及炎性因子升高的量越少。龈沟液分泌量,在20-29岁组,30-39岁组治疗后显著高于治疗前,在40-49岁组,50-59岁组治疗前后差异无统计学意义。白细胞介素1β,各组治疗后1个月均显著高于治疗前。白细胞介素6,各组治疗后1个月均显著高于治疗前。肿瘤坏死因子,在20-29岁组,30-39岁,40-49岁组治疗后显著高于治疗前,在50-59岁组治疗前后差异无统计学意义。结论:钴铬合金烤瓷单冠修复可使各年龄组患者龈沟液分泌量增加,相关炎性因子表达增高。但年龄越大,升高的量越少。
Objective: To investigate the ingredient changes of gingival crevicular fluid after metal ceramic restoration in patients with different ages. Methods: 100 patients (50 male and 50 female, 20-59 years old) indicated for metal ceramic restoration in our department in the past two years were recruited. The patients were divided into four groups (25 patients per group) by age: 20-29, 30-39, 40-49, and 50-59 years old. After routine periodontal prophylaxes, single cobalt-chromium alloy ceramic crowns were restored on each patient. Gross secretory volume of GCF, IL-1, IL-6 and TNF-α levels were measured before restoration and one month after restoration. Result: Compared to those before restoration, the gross secre- tory volume of GCF, IL-1, IL-6 and TNF-α levels significantly increased 1 month after restoration, while the degree of in- crease was inversely proportional to age. The gross secretory volumes (mg/sample) of GCF in the four groups of 20-29, 30-39, 40-49, and 50-59 years old before and after restoration were 0.291 ± 0.023 VS 0.458± 0.057, 0.248 ± 0.031 VS 0.344± 0.048, 0.231± 0.039 VS 0.286± 0.033, 0.267± 0.029 VS 0.277± 0.038; the amount of IL-1 β(μg/L): 7.66± 1.04 VS 18.38± 3.17, 8.35± 1.21 VS 16.85± 2.27, 7.84± 0.95 VS 14.36± 2.86, 8.03± 1.14 VS 13.36± 2.31; the amount of IL-6(ng/L): 27.82± 3.54 VS 66.48± 7.36, 30.74± 2.79 VS 62.47± 6.93, 28.36± 3.67 VS 52.26± 5.49, 31.07± 4.42 VS 39.15± 3.42; the amount of TNF-α (μg/L): 1,12 ± 0.15 VS 2.04 ± 0.26, 1.26 ± 0.20 VS 2.11 ± 0.19, 0.96±0.22 VS 2.24 ± 0.31, 1.06 4- 0.28 VS 1.48± 0.44. Conclusion: Cobalt-chromium alloy ceramic restorations could cause an increase of GCF secretion, and IL-1, IL-6 and TNF-α levels in GCF in the patients with different ages. The degree of increase is inversely proportional to age.
