化腐再生法联合小切口引流治疗糖尿病足急性坏死性筋膜炎

Rot Regeneration Method Combined with Small Incision and Drainage in Treatment of Acute Necrotizing Fasciitis Associated with Diabetes Foot

目的:观察化腐再生散联合小切口引流治疗糖尿病足急性坏死性筋膜炎的临床疗效。方法:将糖尿病足合并坏死性筋膜炎60例患者分为治疗组和对照组各30例。在糖尿病一般治疗的基础上,治疗组采用化腐再生散联合小切口引流、生肌象皮纱条外敷治疗,对照组采用大切口一次性切开换药治疗。结果:治疗后治疗组红肿面积(4.1±5.7)cm2,对照组(10.4±18.9)cm2,两组红肿面积均呈明显下降趋势(P<0.05),并且治疗组下降趋势更明显(P<0.05);治疗组治疗前溃疡面积(3.5±0.2)cm2,治疗后溃疡面积(1.0±0)cm2,对照组治疗前溃疡面积(3.6±0.2)cm2,治疗后溃疡面积(2.0±0.1)cm2,溃疡面积均较治疗前明显缩小(P<0.05),并且治疗组溃疡面积缩小程度更明显(P<0.05)。结论:化腐再生散联合小切口引流治疗急性坏死性筋膜炎合并糖尿病足,可避免组织的过度损伤,使坏死筋膜组织溶解,缩短病程。

Objective Observation of rot regeneration powder combined with small incision and drainage ef-fect in the treatment of acute necrotizing fasciitis and diabetic foot. Methods Sixty patients with diabetic foot necrotizing fasciitis were divided into treatment group 30 cases and control group 30 cases. Based on the general treatment of diabetes, the treatment group was treated with rot regeneration powder combined with small incision and drainage, and Shengji Xiangpi gauze topical therapy; the control group with large disposable incision and dressing therapy. Results After treatment the swelling area in treatment group was （4.1±5.7） cm2, and that in control group was （10.4 ± 18.9） cm2. The swelling area in both groups was significantly reduced（P〈0.05）, the treatment group had a little swelling area（P〈0.05）. Before treatment the ulcer area of treatment group was （3.5± 0.2） cm2, and that in control group was （3.6 ± 0.2） cm2. After treatment the ulcer area of treatment group was （1.0±0.0） cm2, and that in control group was （2.0±0.1） cm2. The ulcer area of both groups was significantly re-duced（P〈0.05）, while the treatment group had a little swelling area（P〈0.05）. Conclusion Rot regeneration with Sanjie combined with small incision and drainage in the treatment of acute necrotizing fasciitis associated with diabetic foot, and can prevent excessive tissue damage, can cause necrosis of fascia tissue dissolu-tion, thus shorten the healing course of the disease.
