运用秩和比法对医院临床科室进行综合评价

A Comprehensive Evaluation to the Clinical Departments of a Hospital with the Application of Rank and Analogy Method

目的运用秩和比法对某三甲医院2013年临床科室工作质量指标进行综合分析和评价,为医院的管理决策提供参考。方法收集某三甲医院2013年临床科室医疗工作质量,组成综合评价指标,利用SAS8.2软件编写语句计算RSR值、计算回归方程,根据RSR分布中的累积概率求probit值进行分档排序,编写ANOVA和SNK语句进行分档排序检验和方差齐性检验。结果对临床各科室综合评价分为3档,经方差分析(F=0.96,P=0.3952>0.05)方差齐和SNK分析(F=61.16,P<0.001)3档差别有统计学意义,小儿内科、产科、骨病科、妇科、甲状腺乳腺外科、骨创科probit值较高(6.41780,6.80274,6.15035,5.57913,6.00999,6.86273,6.41780)提示科室医疗工作质量较高。而肿瘤科、肝胆外科、重症医学科、康复医学科probit值较低(0.30357,4.32551,3.46588,3.13727)提示科室医疗工作质量较低。结论秩和比法灵活简便,对样本资料无特殊要求,在医院多指标综合评价中具有较好的应用价值。

Objectives To conduct a comprehensive evaluation to the work quality indexes of the clinical departments of a Three A Grade Hospital in 2013 with the application of rank and analogy method,thus provide reference for the management decisions of the hospital.Methods To collect the work quality of the clinical departments of a Three A Grade Hospital in 2013 to make up the comprehensive evaluation indexes,calculate RSR score and regression equation with the application of programming sentences in SAS 8.2 software,then seek the probit score to make sub-file according to the cumulative probability in the distribution of RSR,and write the ANOVA and SNK sentences to conduct sub-file rank test and homogeneity test of variances.Results The comprehensive evaluation of each clinical department could be divided into three grade,and there was statistical significance by ANOVA analysis （F=0.96,P=-0.3952＞0.05）,homogeneity test of variances,and SNK analysis （F=61.16,P＜ 0.001）,the probit score of departments such as pediatric medicine,obstetrics,osteonosus,gynaecology,thyroid and breast as well as bone trauma were relatively high（6.41780,6.80274,6.15035,5.57913,6.00999,6.86273,6.41780）,which led to their higher work quality of medical treatment.The probit score of departments such as oncology,hepatobiliary,intensive care and rehabilitation medicine were relatively low,which led to their lower work quality of medical treatment.Conclusions The rank and analogy method was flexible and simple,and had no specific requirements to the sample materials,which had well applicable value in the multiple indexes comprehensive evaluation of the hospital.
