原发性输尿管纤维上皮性息肉的诊断及治疗(附12例报告)

Diagnosis and treatment of ureteral fibroepithlial polyps

目的：探讨原发性输尿管纤维上皮性息肉的诊断及治疗方法。方法回顾性分析2008年4月至2013年10月诊治的12例原发性输尿管纤维上皮性息肉患者的临床资料,结合相关文献总结其诊断方法及治疗措施。结果12例输尿管息肉均于术前通过影像学及活检病理分析得以明确诊断,其中左侧7例,右侧5例,均为单发。息肉位于输尿管上段5例,中段4例,下段3例。息肉长1.4--11.0 cm,平均6.2 cm。开放手术治疗3例,输尿管镜手术9例,手术均获成功,近期无尿漏、感染,远期无输尿管狭窄、闭锁,无息肉复发及恶变。12例输尿管息肉术后病理学检查亦均证实为原发性输尿管纤维上皮性息肉。结论原发性输尿管纤维上皮性息肉为良性肿瘤性病变,亦非癌前病变。影像学检查易于发现肿块及周围病变,活检病理可明确定性,手术干预效果良好。

Objective To explore the method in treating and diagnosing the ureteral fibroepithlial polyps. Methods Combined with the previous reports,the method of diagnosis and the treatment of the ureteral fibroepithlial polyps were analyzed.The clinical data of 12 cases of ureteral polyp from April 2008 to October 2013 was collected and then put it into retrospectively analyzed. Results Radiographic and histologic analysis technics were used to make the clear-cut in all of the 1 2 cases before the operation.Ure-teral polyp was found be left for 7 cases,right for the rest of 5 cases.Polyps were located in the upper ureteral for 5 cases,middle ureteral for 4 cases and lower ureteral for 3 cases.The length of polyp is 1.4 to 11.0 cm,average for 6.2 cm.9 cases were treated with open surgery,3 cases were treated with ureteroscopy,and all of the operations have got a good outcome.Postoperative pathology veri-fies that 1 2 cases was ureteral benign polyp.No urine leakage,infection,ureteral stricture,relapses or malignant changes were found after short-term and long-term follow-up. Conclusions Primary ureteral fibroepithlial polyps is a kind of benign tumor lesions,nor is it a precancerous lesions.It is easy to be finding with the imaging examination and diagnosis with the pathological examination,and the surgical treatment can obtain a good outcome.
