动态血压负荷对原发性高血压患者左室收缩功能不全的预测价值 被引量:4

摘要 目的:观察动态血压负荷(BPL)对原发性高血压左室收缩功能减退的预测价值。方法:根据左室射血分数,将66例原发性高血压患者分为心功能减退组及心功能正常组,比较临床资料及BPL值。30例健康者作为正常对照组。结果:高血压组患者24h,白昼及夜间BPL均显著高于正常对照组(P<0.01)。在高血压患者中,心功能减退组24h,白昼及夜间BPL均明显高于心功能正常组(P<0.05)。BPL≥60%时,预测左室收缩功能减退阳性预测值74.62%,阴性预测值70.15%。结论:动态BPL能够较特异地预测高血压左室功能减退。 Objective:To evaluate the value of ambulatory blood pressure load (BPL) for predicting the left ventricle systolic hypofunetion for patients with primary hypertension. Methods: Based on the left ventricular ejection fraction, 66 patients with primary' hypertension were divided into normal cardiac function group and cardiac hypofunetion group. The clinical data and BPL were analyzed. The control group included 30 healthy individuals. Results:The 24-hour, daylight and nighttime BPLs in hypertension group were significantly higher than those in control group (P 〈 0. 05). In patients with primary hypertension,the 24-hour, daylight and nighttime BPLs in cardiac hypofunction group were markedly higher than those in normal cardiac function group (P〈0.05). When the BPL was equal to 60% or over, the positive predictive value of left ventricle systolic hypofunction was 74.62 %and the negative predictive value was 70. 15%.
