腹膜后淋巴瘤MRI表现探讨

MRI performance of retroperitoneal lymphoma

目的:探讨腹膜后淋巴瘤MRI表现特征。方法:回顾性分析经手术病理证实的10例腹膜后淋巴瘤患者的MRI征象。结果:1部位:上腹部2例、中腹部5例、下腹部3例。2病理证实:非霍奇金淋巴瘤8例(T细胞型淋巴瘤2例、大B细胞型淋巴瘤5例、滤泡细胞性淋巴瘤1例);霍奇金淋巴瘤2例。38例平扫表现肿瘤为均质信号,2例伴有少许坏死。4继发改变:2例侵犯腹主动脉及腹腔干,5例侵犯肠系膜上动脉,2例侵犯直肠后脂肪间隙,1例侵犯髂内动脉、髂外动脉。结论:10例淋巴瘤,8例MRI平扫均表现为均质软组织信号,DWI呈均匀高信号,增强扫描显示轻度-中度均匀强化;2例病灶内伴少量坏死,其余表现同前8例。

Objective： To explore MRI performance of retroperitoneal lymphoma. Methods： MRI performance of retroperitoneal lymphoma confirmed by surgical pathology in 10 cases were analyzed retrospectively. Resuits.（1）The tumors were found in upper abdomen in 2 cases,in mid abdomen in 5 cases, and in lower abdomen in 3 cases. （2）The tumors were proved to be non-Hodgkin＇s lymphoma in 8 cases （T -cell lymphoma in 2 cases, large B-cell lymphoma in 5 cases, and follicular lymphoma in 1 case） and Hodgkin＇s lymphoma in 2 cases through surgical pathology. （3）Homogeneous signal were showed in 8 cases and focal necrosis were accompanied in 2 cases by plain scan. （4）Tumor invasion were found in abdominal aorta and celiac trunk in 2 cases, in superior mesenteric artery in 5 cases, in fat space behind rectum in 2 cases,in internal iliac artery and external iliac artery in 1 case. Conclusion： Among 10 cases of lymphoma, homogeneous soft tissue signals were showed through MRI plain scan with homogeneous high signals in DWI（diffusion weighted imaging）, and with mild to moderate even reinforcement by enhanced scan in 8 cases, while focal necrosis were accompanied in 2 cases.
