液基细胞学联合HPV-DNA检测在宫颈癌前病变中的临床价值 被引量:12

摘要 目的:探讨在宫颈癌前病变筛查中应用薄层液基细胞学联合人乳头瘤病毒HPV-DNA检测的价值。方法根据入选标准,选择2007年4月~2012年6月由本院宫颈科诊断为宫颈癌前病变并收妇科病房住院治疗的患者151例,同时运用HPV-DNA 基因分型检测方法对其中46例进行检测,并对相关数据进行统计分析。结果对151例入选病例进行ROC曲线统计分析,结果表明宫颈病变程度为中度上皮内瘤变及以上病变的病例,其TCT结果和术前病理的曲线下面积存在以下关系:AUC 细胞学=0.603(P<0.05)<AUC 术前病理=0.932(P=0.025<0.05)。对46例进行HPV-DNA检测的病例分别采取TCT、HPV-DNA检测、TCT联合HPV-DNA检测三种方法诊断CINⅡ+,诊断结果进行ROC曲线统计分析,曲线下面积越来越高,数值分别为0.723(P<0.05)、0.772(P<0.05)、0.843(P<0.05)。结论运用液基细胞学联合HPV-DNA检测进行宫颈癌前病变筛查是较为敏感和准确的模式。 Objective To discuss the value of thin-prep cytology test combined with human papilloma virus HPV-DNA detection in screening cervical precancerous lesions. Methods A total of 151 patients with cervical according to the selection criteria by the institute for the diagnosis of cervical precancerous lesion and accept the gynecological ward hospitalization cases from April 2007 to June 2012, at the same time, the use of HPV DNA genotyping method in which the test was performed in 46 patients, the date was analyzed. Results By analyzing of ROC curve on the statistics of 151 patients, the results that the degree of cervical lesions are moderate intraepithelial neoplasia lesions and above, and there are several relations area of the TCT results and preoperative pathology under the curve:AUC cytology=0.603 (P〈0.05)〈 AUCpreoperative pathology=0.932 (P=0.025〈0.05). HPV-DNA detection of 46 cases were taken by TCT, HPV-DNA detection, TCT combined with HPV-DNA detection of three kinds of methods for the diagnosis of CIN Ⅱ+, the analysis and diag-nosis results were ROC curve, curve below value was higher, values were 0.723 (P〈0.05) and 0.772 (P〈0.05), 0.843 (P〈0.05). Conclusion Application of liquid based cytology combined with HPV-DNA detection of cervical precancer-ous lesions screening is more sensitive and accurate model.
