抽回血-脉冲式冲管-正压封管常规维护联合抽吸式冲管预防肿瘤化疗患者经外周静脉置入中心静脉导管堵塞效果观察

Effect observation of preventive tumour chemotherapy patients peripherally inserted central venous catheters pipe blockage by A-C-L general maintenance combined with suction flushing pipe

目的探讨抽回血(A)-脉冲式冲管(C)-正压封管(L)常规维护联合抽吸式冲管预防肿瘤化疗患者经外周静脉置入中心静脉导管(PICC)堵塞效果。方法选择86例肿瘤化疗患者,随机分为2组,即A(抽回血)-C(脉冲式冲管)-L(正压封管)结合化疗间歇期加入抽吸式冲管组(观察组)和A-C-L冲管组(对照组),每组43例。观察两组患者导管堵塞发生情况,以及干预前后焦虑情绪变化情况,并评定生活质量。结果观察组患者发生导管堵塞3例(7.0%),对照组发生10例(23.3%),差异有统计学意义(P<0.05)。干预前,两组患者的汉密尔顿焦虑量表(HAMA)总分以及精神性因子、躯体性因子的得分差异均无统计学意义(P>0.05);干预后,观察组患者的总分及各因子分均低于对照组,差异有统计学意义(P<0.05)。两组患者均无失访现象,两组比较,随访6个月、12个月观察组的生活质量各项指标得分均高于对照组,差异有统计学意义(P<0.05)。结论 A-C-L常规维护联合抽吸式冲管可有效预防肿瘤化疗患者PICC导管堵塞,并可降低患者的焦虑情绪,提高其生活质量,具有较好的临床推广价值。

Objective To discuss effect observation of preventive tumour chemotherapy patients peripherally inserted central venous catheters（PICC） pipe blockage by A（assess）-C（clear）-L（lock） general maintenance combined with suction flushing pipe.Methods Selected 86 cases tumour chemotherapy patients who were treated in The Fourth Hospital of Qinhuangdao,randomly divided them into 2 groups,namely：A-C-L combined with added suction flushing pipe at chemotherapy intermittent period（short for observation group） and A-C-L flushing pipe group（short for control group）,43 cases in each group,observed two group patients＇ pipe blockage occurrence,anxiety emotion change of before and after intervention and followed up 1 year,evaluated lives qualities.Results Observation group patients occurred pipe blockage 1cases（7.0%）,less control group＇s 10 cases（23.3%）,the differences had statistical significances（P〈0.05）.Before intervention,two group patients＇ Hamilton Anxiety Scale（HAMA） total scores and spiritual factors＇,physical factors＇ scores all hadn＇ t statistical significances（P〉0.5）;after intervention,observation group patients＇ total scores and each factor score all lower than control group,the differences had statistical significances（P〈0.05）.Each patients all have been followed up,compared the followed result,followed up 6 mo,12 mo,observation group patients＇ each index of life quality all higher than control group,the differences had statistical significances（P〈0.05）.Conclusion A-C-L general maintenance combined with suction flushing pipe can effectively preventive chemotherapy patients PICC pipe blockage;at the same time,can reduce patients＇ anxiety emotion,improve their life quality,have good clinical popularization value.
