Establishment of Benign Prostatic Hyperplasia with Detrusor Underactivity Model in Guinea Pigs

摘要 目的:建立良性前列腺增生逼尿肌收缩无力(Benign prostatic hyperplasia with detrusor underactivity,BDU)动物模型为相关研究提供实验动物平台。方法:50只豚鼠行手术去势(双侧睾丸切除)后,随机分成实验组(n=30)和对照组(n=20),分别皮下注射50 mg/kg的丙酸睾丸酮(testosterone propionate,TP)和等体积的0.9%氯化钠溶液,隔日1次,连续12周。12周后行尿动力学检查和前列腺组织切片病理观察,将病理符合良性前列腺增生和尿动力学存在逼尿肌收缩无力的豚鼠作为BDU组;病理符合前列腺增生但尿动力学检查逼尿肌收缩力正常或增强的豚鼠作为前列腺增生膀胱代偿组(BPH组)。结果:实验组前列腺腺体、间质增生,平均腺体和间质面积明显大于对照组(P<0.05)。尿动力学检查筛选出实验组中22只BDU(73.3%),6只前列腺增生膀胱代偿组(20.0%),2只出现死亡(6.7%)。BDU组除最大逼尿肌压下降,残余尿量、膀胱容量及膀胱顺应性均增加(P<0.05或P<0.01),与BPH组及对照组比较,差异均有统计学意义。结论:去势后豚鼠皮下注射TP 50 mg/kg,隔日1次,持续12周后行尿动力学检查可筛选出造模成功的BDU模型。 Objective To establish a benign prostatic hyperplasia with detrusor underactivity(BDU) model in guinea pigs for experimental study. Methods Fifty male guinea pigs after surgical castration were randomly divided into two groups, experi, ment group (n = 20) and control group (n = 30). The experimental group was subcutaneously injected with testosterone propionate( TP,50mg/kg, once every other day for 12 weeks). The control group received injection of an equal volume of saline. All guinea pigs were detected by urinary dynamics test and pathological examination at 12 weeks and were further assigned into BDU subgroup and bladder compensated(BPH) subgroup. Results In comparison with control group, prostatic volume was significantly enlarged ( P 〈 0.01 ) with glands and interstitial mean area increasing ( P 〈 0.05 ) in experimental group. BDU was present in 22(73.33% ) guinea pigs,while bladder function was compensated in another 6(20% ) guinea pigs and 2 (6.67%) deaths occurred. In comparison with comrol group and BPH group,the maximal detrusor pressure was significantly decreased( P 〈 0.01 ), the residual urine volume, maximum bladder capacity and bladder compliance was also increased( P 〈 0.05 or P 〈 0.01 )in the BDU subgroup, respectively. Conclusion It is practicable to establish BDU model in castration guinea pig by injecting TP with a dose of 50 mg/kg once every other day for 12 weeks.
