高压氧联合GTR治疗前牙骨缺损疗效评价

Therapeutic Effect of Hyperbaric Oxygen Treatment Combined Guided Periodontal Tissue Regeneration on Patients with Anterior Teeth Bone Deletion

摘要 目的:评价高压氧(Hyperbaric oxygen,HBO)联合胶原膜引导组织再生术(Guided tissue regeneration,GTR)治疗前牙骨缺损效果。方法:将20例重度牙周病患者78颗下前牙骨缺损表面行胶原膜覆盖后随机分为单纯GTR组和HBO+GTR组,分别于术前、术后6周、12周记录探诊深度(PD)、附着丧失(AL)、牙龈萎缩(RES)、牙龈指数(GI)、牙齿松动度(TM)、CBCT下新骨形成参数,并进行统计学分析。结果:HBO+GTR组术后6周其缺损中央及边缘有新骨形成,骨密度增高,PD、AL和TM显著减少;RES和GI指数显著改善(P<0.05或P<0.01)。但单纯GTR组直至12周后才逐渐有新骨形成。结论:高压氧联合胶原膜引导组织再生术能加快牙周组织恢复,加快缺损区骨密度增加及新骨形成。 Objective To evaluate the therapeutic effect of hyperbaric oxygen (HBO) treatment combined guided tissue regeneration on bone defect of anterior teeth. Methods Total 78 cases of low anterior teeth bone deletion in 20 patients with serious periodontal diseases were selected and treated with GTR operation. Then the patients were divided into GTR group and HBO + GTR group according to adjuvant treatment with HBO post-operation. The indexes of probing depth(PD) ,attachment loss( AL), gingival recession( RES), gingival index(GI), tooth mobility( TM ), and new bone formation index under CBCT were all determined at pre-operation,6 weeks post-operation, and 12 weeks pest-operation, respectively. All the above data were statistically analyzed. Results The new bone formation have been obgerved in centre and edge of deletion area in patients of HBO + GTR group at 6 weeks post-operation, meanwhile the bone density was increased. The TM value recovered to normal level, the PD, AL and TM was significanfly decreased, the RES and GI was improved ( P 〈 0.05 or P 〈 0.01 ). But the new bone formation was not observed in GTR group until 12 weeks post-operation. Conclusion GTR combined HBO treatment could speed up the recovery of periodontal tissue, including the increasing of bone density of defect area and the new bone formation.
