摘要
目的:探讨系统性红斑狼疮(Systemic lupus erythematosus,SLE)合并重叠综合征患者甲状腺疾病(Thyroid disease,TD)的患病率。方法:以本院肾内及风湿免疫科2008年1月至2014年5月住院并接受甲状腺功能检查的410例SLE患者为研究对象,根据SLE患者是否合并重叠综合征再分亚组,同期接受甲状腺功能检查的非系统性红斑狼疮的500例健康体检者作为健康对照组,分别比较SLE患者、系统性红斑狼疮亚组与500例健康对照组总甲状腺疾病、原发性甲状腺疾病、甲状腺炎的发病率。结果:SLE患者甲状腺疾病总发病率与健康对照组无显著差异(P=0.246)。410例SLE患者中,合并重叠综合征患者共有156例(38%)。不合并重叠综合征亚组较健康对照组总甲状腺疾病、原发性甲状腺疾病、甲状腺炎发病率低(P均<0.05),但SLE合并重叠综合征亚组较健康对照组总甲状腺疾病、原发性甲状腺疾病、甲状腺炎发病率高(P<0.05)。结论:合并重叠综合征的SLE患者甲状腺疾病发病率较高,临床医师更应注意在合并重叠综合征的SLE患者中筛查甲状腺疾病。
Objective To investigate the morbidity of thyroid disease (TD) in systemic lupus erytbematosus (SLE) patients with overlap syndrome. Methods From January 2008 to May 2014,total 410 patients with SLE hospitalized in department of nephrology and department of rheumatism and immunology of Suizhou hospital were enrolled in this study and undertaken thyroid function examination. According to whether or not overlap syndrome was complicated, the 410 patients with SLE were divided into subgroups. At the same time,500 healthy volunteers without SLE that underwent thyroid function examination were selected as control group. The incidences of total thyroid diseases(TD) ,constitutional TD and thyroiditis in all SLE patients,SLE subgroups and healthy control group were comparatively analyzed. Results The incidences of total TD between SLE patients and healthy controls had no significant difference( P = 0. 246). In 410 patients with SLE, 156 cases accounted for 38% complicated overlap syndrome. Compared with healthy controls, the incidences of total TD, constitutional TD and thyroiditis in SLE patients with overlap syndrome were significantly hihger, but which were all significantly lower in SLE patients without overlap syndrome( all P 〈 0.05 ). Conclusion The higher morbidity of TD was shown in SLE patients complicated with overlap syndrome, suggesting that it was clinically required to screen the TD from them.
