中药离子导入对结核性渗出性胸膜炎的干预作用 被引量:2

摘要 目的:探讨中药离子导入在治疗结核性渗出性胸膜炎中的作用。方法:将60例初治结核性渗出性胸膜炎(中量以上)患者随机分为治疗组(30例)和对照组(30例)。对照组给以全身抗结核药物化疗,胸腔闭式引流,胸水不能定位时胸腔注射尿激酶直至再无胸水引出时拔管。治疗组在此基础上,在胸水不能定位时配合中药离子导入治疗。记录两组患者胸腔积液引流的总量,胸水完全消失的时间,并于第30天、第60天复查胸部螺旋CT测量壁层胸膜厚度。结果:两组患者胸腔积液引流的总量无显著统计学差异(P>0.05);治疗组患者胸水完全消失的比例及时间均小于对照组(P<0.05),治疗组患者第30天、第60天壁层胸膜厚度均小于对照组(P<0.05)。结论:中药离子导入可提高结核性渗出性胸膜炎的治疗效果,并对胸膜形态有改善作用。 Objective: To explore the effect of traditional Chinese medicine iontophoresis in the treatment of patients with tuberculous exudative pleurisy. Methods: 60 patients with tuberculous exudative pleurisy were randomly divided into the treatment group( n = 30) and the control group( n = 30). All of the patients were treated with the systemic chemotherapy of anti-tuberculous drugs and closed drainage of thoracic cavity and urokinase was injected into thoracic cavity when ultrasound showed hydrothorax couldn' t be located,and then exuberated when hydrothorax disappeared. On the foundation,the patients in treatment group received traditional Chinese medicine iontophoresis when hydrothorax disappeared. The total amount of hydrothorax and the time of hydrothorax disappear were recorded between these two groups and pleural thickening was also observed on the 30 th and 60 th day on the basis of chest CT for all patients. Results: The total amount of hydrothorax was not significant between these two groups( P〉0. 05). The time and the ratio of hydrothorax disappearance in treatment group were both superior to those of controls( P〈0.05). Pleural thickenings on the 30 th and 60 th day in treatment group were both superior to those of controls( P〈0. 05). Conclusion: Iontophoresis of traditional Chinese medicine can improve the curative effect and improve pleural form in the treatment of patients with tuberculous exudative pleurisy.
