不同方法测量甲状腺上动脉峰值流速与甲状腺功能亢进症诊断的符合率 被引量:13

摘要 目的:探讨不同方法测量甲状腺上动脉(STA)峰值流速与甲状腺功能亢进症(甲亢)诊断的符合率。方法:127例甲状腺弥漫性病变患者,甲状腺血供分级均达到Ⅱ级和Ⅲ级。测量双侧STA的峰值流速,文献传统的测量方法可总结为上段测量法及下段测量法,本研究新增加筛选测量法。将上述三种方法测得的峰值流速记录存档并与实验室指标及临床诊断进行对照。结果:左、右甲状腺上动脉之间的峰值流速可以存在很大的侧别差异;同一侧甲状腺上动脉其上段和下段的峰值流速大多也存在明显的部位差异,多数上段流速高于下段。进一步与实验室指标对照发现,三种测量方法中,筛选法测得的峰值流速与甲亢的符合率最高(80.31%),传统的上段法和下段法符合率依次分别为63.78%、44.88%,三种方法比较差异有显著统计学意义(均P<0.01)。结论:获取甲状腺上动脉峰值流速不应人为指定测量部位,应从动脉全程筛选最大流速。筛选法测得的峰值流速与甲亢诊断的符合率明显优于传统测量方法。 Objective To explore the conincidence rate of diagnosis of hyperthyroidism in measuring peck systolic velocity (PSV) of the superior thyroid artery (STA) using different methods of untrasound. Methods A total of 127 patients with thyroid diffuse diseases was enrolled in the study, in which the thyroid blood supply of all cases reached grade U and grade m. The PSV was measured in bilateral STA by traditional methods in literatures including upper measurement method and lower measurement method.This study added a screening measurement method. The PSV data obtained with the above three methods was recorded, and archived for comparing them with laboratory tests and clinical diagnosis. Results PSV between the left and right STA could be varied. Also, there were evident differences in PSV of the STA in the upper part and lower part of the same side. Usually, the upper velocity was higher than the lower one. Contrasted to the laboratory tests, the conincidence rate of the PSV of hyperthyroidism was 80.31% by screening measurement method, 63,78% by traditional upper part method and 44.88% by traditional lower part method respectively (P 〈 0.01 ). Conclusion To obtain PSV of the STA, we should measure the whole length of the STA, but not in a local point or points. The PSV by screening measurement method has much more higher conincidence rate than traditional methods in hyperthyroidism.
