摘要
目的:探讨锥形束CT(CBCT)在侧壁开窗上颌窦底提升中的临床应用。方法:将上颌后牙缺失,经CBCT检查需行侧壁开窗上颌窦底提升的患者57例(男38例,女19例,年龄29~67岁)纳入研究。CBCT扫描条件120k V,5m A;FOV 16cm×10cm,层厚0.3 mm,完成多维度的多层面重组以明确上颌窦解剖形态及黏膜情况,以最终手术结果作评价。结果:CBCT发现4例上颌窦内出现分隔而进行双开窗手术,57例患者共植入155枚ITI种植体,其中同期植入种植体117枚,延期植入种植体38枚。随诊显示所有种植体骨结合良好,成功率100%。结论:CBCT检查对侧壁开窗上颌窦底提升术有重要指导作用。
Purpose: To investigate the clinical application of cone-beam computer tomography in lateral window sinus floor elevation. Methods: In this study, 57 patients (38 men and 19 women with age range of 29-67), whose maxillary posterior teeth were missed, were decided to receive lateral window sinus floor elevation after CBCT scan. The parameters of CBCT scan were 120kV and 5mA with FOV of 16cm^10cm and layer thickness of 0.3mm. Multi-dimensional multi-level reconstruction was accomplished to make clear display of anatomic characteristics of the maxillary sinuses and conditions of the mucous membranes. The reliability of CBCT was evaluated by the outcomes of the surgical treatments. Results: Four patients received double windows approaches because of maxillary sinus septa detected by CBCT. In 57 patients, 155 ITI implants were placed, of which 117 implants were placed immediately and 38 implants were placed delayed. Follow-up examinations indicated all implants had good bone integration with 100% success rate. Conclusion: CBCT imaging plays an important role in guiding lateral window sinus floor elevation.
