创伤性湿肺胸片与CT平扫的对比研究 被引量:4

摘要 目的:比较胸片与CT平扫对创伤性湿肺的诊断价值。方法:回顾性分析经临床治疗证实的68例创伤性湿肺患者。全部病例均经胸片检查及胸部CT检查。影像诊断与临床治疗对照,比较二者对创伤性湿肺的检出情况及诊断准确率。结果:68例中,胸片检出65例,CT平扫检出68例,二者结合检出68例,胸片的诊断正确率95.6%,CT平扫的诊断正确率10 0%。本组病例中胸片漏诊3例间质型患者。结论:CT对间质型者的病灶显示优于胸片。但立位胸片对肋骨骨折及肺撕裂伤病灶内的液平显示优于CT。胸片因其简单、方便成为创伤性湿肺影像学诊断的首选检查方法,对于病变的探测和显示CT扫描优于胸片。二者结合应用可以提高创伤性湿肺的诊断正确率。因此,CT扫描是胸片诊断创伤性湿肺的重要补充,对已经确诊的创伤性湿肺患者可进行胸片监测复查。 Purpose: To compare the diagnosis value of X-ray and CT scanning for traumatic wet lung. Methods: The data of 68 patients with traumatic wet lung which were confirmed by clinical treatment, were retrospectively analyzed. All the patients were performed X-ray examination and chest CT scanning. By comparing the results of imaging diagnosis and clinical treatment, the diagnostic accurate rates of X-ray and CT scanning for traumatic wet lung were obtained. Results: The diagnostic accurate rate of X-ray for traumatic wet lung was 95.6% (65/68), while CT was 100% (68/68). Three patients with interstitial type lesions were missed-diagnosed by X-ray. Conclusion: CT is superior to conventional X-ray examination for diagnosis of interstitial type lesions. However, erect chest radiograph is superior to CT for the display of fluid level in patients with rib fracture or lung laceration lesions. Chest X-ray examination is the first-selected method for the imaging diagnosis of traumatic wet lung because of its simple and convenience. For the localization and display of lesions, CT scanning is superior to conventional X-ray examination. The combination of CT scanning and X-ray examination can improve the diagnosis accurate rate of traumatic wet lung. Therefore, CT scanning is an important supplementary method for the X-ray diagnosis of traumatic wet lung, and the confirmed patients with traumatic wet lung might be monitored by chest X-ray examination.
