多b值DWI序列对脊柱结核早期评价的动物实验研究 被引量:2

摘要 目的:探讨多b值弥散加权成像及感兴趣区设置方法的选择对早期脊柱结核的应用价值。方法:将动物模型分为病变组(40只)、对照组(15只),术后4周、6周、8周后分别对两组进行多b值DWI检查,观察两组椎体表观弥散系数(ADC值)并拟合ADC图,并通过同一面积单个感兴趣区及多个感兴趣区的选择制定受试者工作特性(ROC)曲线确定临界点,比较两种方法所测得的ADC值诊断脊柱结核的灵敏度、特异度。结果:1病变组第4周ADC3b值、ADC7b值及ADCperf值分别与6周、8周的ADC3b值、ADC7b值及ADCperf值之间存在统计学差异(P<0.05)。2单个ROI测得的ADC值较多个ROI低,但差异没有统计学意义(P>0.05);单个ROI或多个ROI时病变组与对照组的ADC值差别都有统计学意义(P<0.05),多个ROI测得的ADC值诊断脊柱结核的灵敏度、特异度高于前者。结论:MRI能够为脊柱结核早期诊断提供一定信息,其中MRI的多b值弥散加权成像扫描方法及多个ROI测得的ADC值联合应用,在脊柱结核的诊断中更有价值。 Purpose: Explore the multiple b value DWI and setting method to select a region of interest for early application of spinal tuberculosis. Methods: divided animal models into two groups(40, 15), then examined with imaging and pathology methods at 4, 6, 8 weeks respectively. Calculated ADC values and fit ADC maps, and to develop receiver operating characteristic (ROC) curve through the same area of a single region of interest and to select multiple regions, compare the two methods measured ADC values diagnosis of spinal tuberculosis sensitivity and specificity. Results: (1)4 weeks ADC3b, ADC7b and AOCperf values had statistical differences with 6, 8 weeks in the lesion group (P〈0.05).(2)ADC value of a single ROI measured multiple ROI is low, but the difference was not statistically significant (P〉0.05).single or multiple ROI ADC values ROI difference lesion group and the normal group were statistically significant (P 〈0.05), Multiple ROI measured ADC values diagnosis of spinal tuberculosis sensitivity and specificity than the former. Conclusion: MRI can provide some information for the early diagnosis of spinal tuberculosis, multiple b value MR-DWI scanning methods and ADC values of multiple ROI measured in combination, in the diagnosis of spinal tuberculosis is more valuable.
