经乙状窦后入路听神经鞘瘤手术并发症分析及其处理

Complication Analysis and Treatment of Auditory Nerve Sheath Tumor Operation Through Retrosigmoid Approach

目的探讨经乙状窦后入路听神经鞘瘤手术并发症分析及其处理。方法总结50例经乙状窦后入路听神经鞘瘤手术患者术后并发症的发生情况,并分析发生原因及处理方法。结果经乙状窦后入路听神经鞘瘤手术患者主要术后并发症包括脑水肿、脑血肿(18.00%)、呼吸道并发症(10.00%)、面神经麻痹(30.00%)、颅内感染及脑脊液漏(8.00%)以及切口区皮下积液(2.00%)等。通过开颅血肿清除术、激素使用、鼻饲、抗生素使用、康复训练、清洁换药等处理,50例手术患者均康复出院。结论根据经乙状窦后入路听神经鞘瘤手术可能发生的并发症进行预案,在并发症早期阶段进行合理的干预,能够有效降低并发症带来的不良后果。

Objective To discuss and analyze the complications of auditory nerve sheath tumor operation through retrosigmoid ap- proach and the corresponding treatment. Methods The postoperative complications of 50 cases underwent auditory nerve sheath tumor operation through retrosigmoid approach were summarized, and the causes and corresponding treatment were analyzed. Re- suits The postoperative complications of patients with auditory nerve sheath tumor operation through retrosigmoid approach mainly included encephaledema and cerebral hematoma（18.00%）, respiratory tract complication（10.00%）, facial paralysis（30.00%）, intracra- nial infection and cerebrospinal fluid leakage （8.00%）, incision subcutaneous effusion （2.00%） and so on. All the 50 patients under- went the operation were discharged from hospital upon recovery by the treatment of craniotomy evacuation of hematoma, administra- tion of hormone, nasal feeding, use of antibiotics, rehabilitation training, clean and dressing and so forth. Conclusion Pre-arrange- ment based on the complications may be caused by auditory nerve sheath tumor operation through retrosigmoid approach and rea- sonable interventions in the early stage of complications can reduce the adverse consequences of the complications effectively.
