霉菌性鼻窦炎患者经鼻内镜手术治疗的效果观察

Observation on the Effect of Nasal Endoscopic Surgery on Fungal Sinusitis

目的研究霉菌性鼻窦炎患者经鼻内镜手术治疗的效果,为临床治疗提供可参考依据。方法选取该院2011年5月—2013年8月收治的霉菌性鼻窦炎患者200例,作为该次试验的研究对象。按照患者以及家属意愿,分为研究组和对照组,每组100例。研究组采用鼻内窥镜手术治疗,对照组采用传统的手术治疗。统计、比较两组患者不同手术方法的疗效、嗅觉评分和鼻窦炎特异性生活质量量表(SNOT-20)评分,并随访一年,对比复发率。结果研究组总有效95例,总有效率95%,对照组总有效79例,总有效率79%,研究组治疗效果明显优于对照组,差异有统计学意义(P<0.05);与手术前对比,两组患者手术后嗅觉评分均明显上升,SNOT-20评分均明显下降,但研究组改善效果更明显,差异有统计学意义(P<0.05);研究组一年内复发率明显低于对照组,差异有统计学意义(P<0.05)。结论采用鼻内窥镜手术治疗霉菌性鼻窦炎临床疗效显著优于传统手术方法,有利于提高治疗效果,改善患者嗅觉和生活质量,降低复发率,值得推广。

Objective To study the effect of nasal endoscopic surgery on fungal sinusitis so as to provide a reliable reference for clinical treatment. Methods 200 patients with fungal sinusitis were selected as the subjects of this experiment. And they were divided into the research group and the control group with 100 cases in each group according to the wishes of the patients and their families. The research group were treated by the nasal endoscopic surgery, and the control group were treated by the traditional surgical treatment. The curative effect, olfactory score and Sino-nasal Outcome Test-20（SNOT-20） score of the groups were count-ed and compared. All the patients were followed up for 1 year, and the recurrence rate of the groups was compared. Results In the research group, the treatment was effective in 95 cases, the total effective rate was 95%, in the control group, the treatment was ef-fective in 79 cases, the total effective rate was 79%, the treatment effect of the research group was much better than that of the control group, the difference was statistically significant（P0.05）; compared with before surgery, the olfactory score of both groups increased and SNOT-20 score decreased significantly, but the improvement of the research group was more obvious, the difference was statistically significant（P0.05）; the recurrence rate of the research group 1 year after surgery was much lower than that of the control group, the difference was statistically significant（P0.05）. Conclusion For fungal sinusitis, nasal endoscopic surgery has much better clinical effect than traditional surgical method, which is conducive to improving the treatment effect and the sense of smell and quality of life of the patients and reducing the recurrence rate, so it is worthy of promotion.
