微创保胆取石术与腹腔镜胆囊切除术治疗胆囊结石的疗效对比

Comparison of curative effect of lithotomy and laparoscopic cholecystectomy for gallstone minimally invasive gall preserving

目的探讨微创保胆取石术与腹腔镜胆囊切除术治疗胆囊结石的临床疗效。方法选取2012年9月—2013年6月我院收治的80例胆囊结石患者作为研究对象,随机分为观察组与对照组,每组40例。观察组采用微创保胆取石术,对照组则采用腹腔镜胆囊切除术。对比两组患者的手术时间、术中出血量、术后首次肛门排气时间、术后镇痛治疗、术后住院时间、切口感染以及术后随访的情况。结果经手术,观察组与对照组相比,术后恢复良好,手术时间与住院时间比较短、出血量比较少、术后排气时间较短、术后疼痛较轻、费用较低,且观察组的并发症发生率明显低于对照组(2.5%vs48.8%),P<0.05,有统计学意义。结论微创保胆取石术能够有效减少出血量与并发症的发生,缩短手术时间与住院时间,节省住院费用,提高生活质量,值得临床推广。

Objective To investigate the clinical effect of minimally invasive gallbladder-preserved cholecystolithotomy and lap-aroscopic cholecystectomy in patients with gallstone.Methods Eighty patients with gallstone were selected from 9th,2012 to 6th, 2013 and randomly divided into observation group and control group with 40 cases in each.The patients in the observation group were treated by mini-cholecystolithotomy,while in the control group laparoscopic cholecystectomy treatment was used.The opera-tive time, blood loss, postoperative passing gas time, postoperative pain, the days in hospital,complications and hospital costs of the two groups were analyzed.Results The patients in the observation group recovered well and there were no seriouscomplications, and no stones were found in gallbladder and common bile duct through B ultrasound examination. The operation time, the time in hospital and the cost of the patients in the observation group were shorter and lower （P﹤0.05） compared with those of the patients inthe control group. Twoyears of follow-up witnessed no stone reoccurrence, bile duct stricture or cancerous change.Conclusion MC has advantages of shortening the operation and hospitalization time, reducing blood loss during operation, decreasing the com-plications and patients＆#39;costs in hospital and improving the quality of their lives as well.It is worthy of clinical promotion.
