Preliminary research of application of magnetic resonance elastography in meningioma

摘要 目的 验证磁共振弹性成像(MRE)评价脑膜瘤质地的准确性,为神经外科医生提供判断脑膜瘤质地的方法.方法 选取2009年9月至2010年3月于首都医科大学附属北京天坛医院神经外科住院的脑膜瘤术前患者14例.所有受试者均行MRE扫描,将所得原始图像经计算机处理后,分别测得患者正常区域大脑灰质、白质以及肿瘤组织的剪切模量.将肿瘤质地的测量结果与术者在术中对肿瘤质地的评判进行比较.结果 所有患者均成功获取正常区域大脑灰质、白质以及肿瘤组织的剪切模量.大脑灰质(4.839±0.971)kPa,大脑白质(10.249±1.870)kPa.3例肿瘤弹性值小于白质,为质软;8例肿瘤弹性值等于白质,为质中;3例肿瘤弹性值大于白质,为质硬.MRE对肿瘤质地的判断与术中评判一致,符合率为100%.结论 MRE评价脑膜瘤质地的准确性得到初步验证,它能为该肿瘤术前评估及患者预后提供重要的信息. Objective To verify the accuracy of magnetic resonance elastography (MRE) in assessing the elasticity of meningioma,and to provide a more reliable way to evaluate the consistency of meningioma for neurosurgeon.Methods Fourtecn consecutive patients with meningioma hospitalized in neurosurgical department of Beijing Tiantan hospital from September,2009 to March,2010 were involved.All the subjects underwent head MRE.After postprocessing raw images by computer,the shear modulus of grey matter and white matter of normal brain tissue,and shear modulus of tumors were seperatively messured.The assessment of tumor stiffness by MRE was compared to that by neurosurgeons.Results All the shear modulus of grey matter and white matter of normal brain tissue,and shear modulus of tumors were successfully obtained.The shear modulus of grey matter and white matter was(4.839 ± 0.971) kPa and (10.249 ± 1.870) kPa respectively.Among all patients,3 tumors were soft with shear modulus smaller than that of white matter,8 tumors were medium with shear modulus equal to that of white matter,and 3 were hard with shear modulus larger than that of white matter.The elasticity of tumors was agreed with those given by neurosurgeons in all cases.Conclusion The accuracy of assessment of meningioma stiffness by MRE was preliminarily proved.MRE could provide important information for the and prognosis.
