去铁敏治疗自发性脑出血临床初步观察 被引量:2

Effects of deferoxamine in patients of spontaneous intracerebral hemorrhage

摘要 目的评价用去铁敏治疗临床自发性脑出血患者的安全性和疗效。方法临床自发性脑出血病例共29例进入观察序列,其中去铁敏治疗组19例,对照组10例,发病后静脉注射去铁敏针剂,发病第1、7和14天行脑部MRIT2WI和T2%wI扫描,ImageJ软件进行图像分析,发病第7、14和90天行神经功能评估。结果患者使用去铁敏后,未出现药物相关不良反应。患者脑出血后第1、7和14天治疗组脑组织相对水肿程度(1.08±0.57、1.37±0.68、1.52±0.69)比对照组(1.89±0.77、2.03±0.71、2.07±0.87)明显减轻(P〈0.05);脑出血后第7和14天治疗组相对血肿体积(0.96±0.21、0.89±0.23)比对照组(1.16±0.23、1.24±0.49)明显缩小(P〈0.05);脑出血后第7和14天治疗组患者神经功能(11.7±4.1、7.4±2.6)比对照组(15.1±4.9、11.8±5.6)明显好转(P〈0.05)。结论去铁敏对临床自发性脑出血患者的治疗有明显的效果。 Objective To explore the therapeutic results of deferoxamine in the treatment of spontaneous intracerebral hemorrhage (SICH). Methods A total of 29 spontaneous intracerebral hemorrhage patients were collected from 4 clinical centers in Shanghai and Ningbo City. Nineteen cases received deferoxamine treatment as the treatment group, and 10 cases received routine therapy as the control group. Deferoxamine was given after the onset of stroke for a consecutive of 3 days. All cases had brain MRI T2WI and T2 * WI scanning at the 1st, 7th and 14th days after the onset. The image features were recorded and image analysis was performed by the software of Image J. The neurological function was evaluated at the 7th, 14th and 90th days after the stroke. Results Patients in the treatment group didn't show any drug related side effect. The cerebral edema in the treatment group patients with at the lst,Tth and 14th days ( 1.08 ±0. 57,1.37 ±0. 68,1.52 ± 0. 69) were lighter than those in the control group( 1.89 ±0. 77,2. 03 ± 0. 71,2. 07 ± 0. 87) ( P 〈 0. 05 ). The hematoma volumes of the treatment group patients at the 7th and 14th days (0. 96 ±0. 21,0. 89 ±0. 23) were smaller than those in the control group ( 1.16 ±0. 23,1.24 ±0. 49) ( P 〈 0. 05). The neurological function in the treatment group patients at the 7th and 14th days after SICH ( 11.7 ± 4. 1,7.4 ± 2. 6) was better than that in the control group ( 15. 1 ± 4. 9,11.8 ± 5.6) ( P 〈 0. 05 ). Conclusion Deferoxamine had obvious therapeutic effect on the clinical treatment of patients with SICH.
